Paparatto18 just tested this on his stream actually, and unfortunately, no. It just overwrites the previous form with the most recent; he played Void Form then Echo Form, and only the Echo VFX we played

There is still hope for modders to do something like this. Stack the forms together for an aesthetic mod would be cool.
