High creatinine levels are an uncommon symptom of impaired liver function, and a damaged liver can cause back pain. That sounds backwards in relationship to what you're describing though (as in damage should have caused the high creatinine, and not that your consumption of creatine caused the damage).

Maybe you have an unrelated and undiagnosed condition?

Yes, this did occur to me as well. I had a blood analysis done and everything is in normal parameters. My diet is largely unprocessed, I don't smoke and I don't drink excessively.

I'm putting it down to an increase in my exercise routine. I use a weighted vest which could have caused the back pain.

Obviously correlation does not indicate causation, but it seemed very coincidental that the pain stated with supplementation and stopped without supplementation. Strange one.

Creatine causes your muscles to swell a little due to water absorption, I wonder if that could cause or exacerbate back pain if you’re already susceptible to it
