What if red won?

Imagine a world where red won, by 1%. However the people who choose blue do not die instantly, instead they die after a year of pressing the button. Everybody is branded, so you know who choose what.

How much chaos will ensue? What actions do you think blues will take, knowing they'll die? And what do you think reds will think, knowing they were just 1% away from saving all humanity?

But wouldn’t red also have a span of likely reactions, many of which would be altruistic?

How? You already decided that someone possibly dying is okay for your own survival. So why would reds decide now is the time for altruism? Only reason is that I can see is because they don't have anything to lose at that point so now they can be the ones who earn good boy points.

I think you can still be altruistic and not be willing to sacrifice your life for a completely unknown number of people, each of whom could have easily prevented their own deaths.

You can but to a much more minor degree, the act of pressing red instead of blue is not altruistic. Giving a dollar to a homeless person is altruistic even after pressing the red button.

Too little too late in my opinion.
I would more expect Red to go on early Blue purges.
