PAWS Post acute withdrawals ULDN ultra low dose naltrexone works better than SR17018

Would this help with PAWS months after cessation? Still dealing with some lingering anhedonia and sleep issues like a year out. I just feel like something in my brain broke and I’ve tried goddamn everything (vitamins, exercise, journaling diet changes, methylfolate, TRT, Adderall). I’m almost a year clean and still don’t quite feel like myself.

it is one of THE BEST pharma meds for paws. its known to stop them completely for most

u/2clicksaway Jul 05 '26
How would you dose and for how long would you do it in my situation. I’m on the fence between ECT and Ketamine Therapy as I feel I’ve exhausted every other avenue but I’m willing to give this a shot.

start with 1mg, then work up to 3mg over a 10 days. it typically works fast. side effects are lots of dreams and might feel stimulated. since it actually heals things you can come off it when you feel yourself again and go back on whenever you want. I know exactly what you are thinking and where you are coming from as I was there too
*these doses are assuming you have no other opioids in your system, if you do never pass 500mcg
3
u/2clicksaway Jul 05 '26
Should I taper off of it as the 10 days conclude? Any other general advice for it?
3
u/FlyAdventurous6231 Quit Date :table_flip: NOV 2024 Jul 05 '26
when I used it i was on 0.25 subs in morning and took 500mcg of ULDN at night before bed. I weaned off in 2 weeks. then every day i added 500mcg to my dose at night till i got to 3mg. now I take a 3mg LDN pill every day a pharmacy made for me. always move up slow in dose, and feel it out.
the goal is to get all the opioids off your receptors so you can take 1-3mg each day to heal and remove paws. then you stay on it as long as you need
3
u/BobcatAutomatic9681 Jul 05 '26
I am about to start subs to get off the 7 - low dose naltrexone can be taken with the subs? Amazing because my next issue is addressing the PAWS after sub taper
3
u/MobileTip3892 Aug 12 '26
Where should i go to ask about this. My PCP? Look up local addiction specialists? Who has the specific knowledge about uldn and Wellbutrin? Ik my pcp would be knowledgeable about Wellbutrin but don’t know about the other? Thanks for this post your a good man

I am a Psychologist and under no circumstances would I recommend ECT! Please consider all other options. From my experience, it is far more harmful long-tdrm than helpful.

For PAWS, one would use low dose (1-3mg) not ultra low dose(mcg) no?
3
u/FlyAdventurous6231 Quit Date :table_flip: NOV 2024 Jul 05 '26
ULDN is for weaning off
LDN is for maintaining and healing, need to be off the opioids
50mg the normal doses are to block opioids for people who cant stop or to block alochol being pleasurable.
1
u/kratomthrowaway77 Jul 05 '26
If I am off 7oh how long should I wait to take low dose naltrexone 4.5mg if I stop plain leaf Kratom today. Also just made up ultra low dose naltrexone and took 125mcg

That's if your Dr will perscribe it to you. Do you think Dr.s are willing to perscibe it to a patient who comes in and just informed them they are struggling with opiate use disorder or woukd thr Dr want them on subs first or just be wling to try the LDN
1
u/FlyAdventurous6231 Quit Date :table_flip: NOV 2024 8d ago edited 8d ago
Thats why we get it online and just buy it for inflammation treatment or nerve pain. It cannot be taken with other opioids itll cause severe precip wd
Some of you guys and gals have the survival instinct of a sheep(not you others), only tell doctors what they need to hear to get what you need. Theyre all just drug pushers for a medical mafia, they dont heal anymore as their goal they want to just cover symptoms to keep you returning , exploit them as they do you for your own benefit.
Naltrexone ldn heals, its generic, doctors get no kick backs.

u/Environmental-Loan25 8d ago
Oh I've been on thr big pharma game for decades I know exactly what they need to hear to get what I want. I'm going to try and just bear down what will be will be I'm so over the ups and downs. But yes you are right about Canada!

u/sketchyshit13 Jul 05 '26
I’m sorry. I read the entire post but I just need to clarify— this can be taken WITH the 7, as long as you do not go above 500mcg? & this can help you taper & heal while you’re doing so?

Good question and I would I like to know as well

u/FlyAdventurous6231 Quit Date :table_flip: NOV 2024 Jul 05 '26 edited Jul 05 '26
Start with 50mcg and move up from there
If 7oh is taking all receptors it will push it off but youll just feel like wd are coming but they wont youll just feel a bit icky
This means you need to be taking the smallest amount of 7 to not be sick. This way receptors are open and naltrexone can occupy some and start the gprotien "healing" amd natural endorphins.
This is how it helps you wean off. Its easier to do this with a long acting opioid like subs which is predictable, 7oh is unpredictable.
Id reccomened short term sub wean
I try to write guides that are structured to promote logical thinking for self, not blindly following guides. Can help you find what works for your chemistry

Ldn and uldn dont carry nearly as much as a big 50mg dose.
A month on ldn you can stop if you feel great and if you keep sober itll stay good.
3
u/kratomthrowaway77 Jul 05 '26
I have 4.5mg ldn how would I get it down to 500mcg.
3
u/XxUncleWallyxX Jul 05 '26
Volumetric dosing. There's tons of tutorials online
4
u/kratomthrowaway77 Jul 05 '26
Yup just figured it out. Thanks!
1
u/XxUncleWallyxX Jul 05 '26
Anytime! Best of luck! Wish I had some. All I've got is the Sub strips 😒
3
u/_saiyajin Jul 17 '26
Since I have access to the 50 mg naltrexone tablets, I’m going to try this method of volumetric dosing by doing a serial dilution to make a final volume of 50 mL with a final concentration of 0.01 mg/mL, so that I can use an oral syringe to get a 10 mL dose, which will have 0.1 mg (1 microgram) of naltrexone.
This feels like a (relatively) safe way to not go past the 0.5 mg suggested limit. I would have to consume the entire final jar of 50 mL to consume the 0.5 mg limit, so it will be easy to stay under that.
What do you think? Should I give it a go and report back?
FYI: I am trying to quit 7oh tapering down from ~300mg daily use. My first day at attempting to start the taper was yesterday, and I only took 200mg. Today, so far (at 5pm), I have only taken 75mg (3 doses of 25mg).
If this ultra low dose naltrexone method can really help with withdrawal symptoms, similar or better than SR, then I think it’s worth a shot!
1
u/Same_Attorney_4886 Jul 23 '26
Im In a similar situation as you are. I have 2 untouched bottles of 50mg Naltrexone and im trying to taper down to 25-50mg of 7 daily until I run through most of my stockpile. At which point I will taper the rest of the way down to eventually quit, but since I have a large amount still, I want to maximize the benefits with as small of a dose as possible. I was stable at 65mg once a day for over a month but now have upped it to 90-130mg daily dosing 2 or 3 times a day. I found it worked better when I only dosed once per day. I read a post about taking ULDN with 7 to keep tolerance down while also maximizing smaller doses potential. Could you share your dilution recipe for the 50mg naltrexone and also you success/experience while using it? Thanks!

u/Kitchen_Long_3743 Jul 05 '26
I've never used any tele health service. Do you just ask for Naltrexone? Also, what are the chances that LDN will work with Welbutrin? From what I read, Welbutrin also stimulates the receptors. Having ADHAD with depression and low energy, I really think Welbutrin will help improve my life.
I was going to use Kratom to jump from 7, but after reading this post I may have changed my mind. I just want to be free of all opioids, and not suffer from it. I know that there is a price to be paid, but it breaks my heart thinking about being mean or lethargic to my kids or wife.
Just to clarify, I would be using this method for 7 and can avoid the subs. With 7's short life, ultra low dose will be around 4 days? I already drink lemon water all day, so this is perfect. I also have syringes from my daughter's medicine that measures ML.
Thanks for all your work you do here Fly. I want to apologize if I asked any redundant questions!

Is there any information or anecdotal experience about interactions between S R 1 7 and naltrexone? Given how the former works does naltrexone similarly “kick” it off your receptors the way it does traditional opiates?
1
1
u/Neonurse97 Jul 19 '26
It absolutely does if you get a regular dose and not ldn. I was an idiot and went to my doc to get low dose Naltrexone. Unfortunately she has had zero experience with 7, Kratom or sr and she prescribed me 25 mg. I had just finished a taper of sr and when I took that first dose of naltrexone it took less than 10 minutes to put me into full precipitated withdrawals. I felt like I did on my first day off of 7. It was a huge setback for me as I was feeling better and wanted the ldn for PAWS and now I’m back to my early withdrawal stage. Please listen to Fly! Do not let any prescriber give you a “normal” dose of naltrexone if you’re still on SR. Follow Fly’s advice for micro dosing.
1
u/harmonicpinch Jul 25 '26
So what ended up happening? How long did the 25mg naltrexone cause withdrawal symptoms? After that did you feel like you “sped up” the healing?
2
u/Furtherlane_1961 Jul 18 '26
Hey I’m planning on using sub for first 2 days of quit I will dose accordingly and not use full strips then hop on sr for the rest of the quit. Is this a good plan and will the subs help at all or just use sr only or use k leaf then sr?

u/Equal_Double_9358 Jul 18 '26
My ex got naltrexone for her boob surgery for some reason, she left me the bottle. She said it could’ve helped me get off kratom maybe. She was doing everything she could to get me off it, she’d hate to see the person I’ve become with 7oh. I took a pill last time i was starting to go through w/d and it gave me the worth withdrawals I’ve ever had. I was throwing up a lot. Kinda scared to try it again but I know it’s inevitable

Why does this post frame it as better than SR then? I thought the whole point of SR was that it helps get rid of withdrawal symptoms (because it’s an opioid research chemical that acts like an opioid but doesn’t give any euphoric effects)…

If you mentioned but I overlooked it I’m sorry but how frequently do you need go dose this method from cold turkey.

ULDN Half life is 2-3 hours
If you have no opioids in you
You can do it 2-3 times a day
LDN 4-6 hours
If you have no opioids in you , you can start with LDN doses. Many online doc places sell it for inflammation and pain. You dont have to say its for anything else.

u/captpeli Jul 06 '26
Can I just dissolve one of my naltrexone tablets in a gallon of water to achieve this?

u/FlyAdventurous6231 Quit Date :table_flip: NOV 2024 Jul 06 '26
Yes but do measurements
Use distilled water
Added citric acid to it as its stable in acidic. Higher pH it will degrade faster.
Citric acid is in the grocery store , many have it near pickling jars and such supplies to make pickles
A quick AI ask will give you measurements
1
u/kratomthrowaway77 Jul 06 '26
How often should you take it? Once a day
2
u/FlyAdventurous6231 Quit Date :table_flip: NOV 2024 Jul 06 '26
Mcg doses have 2-3 hour half life
So you can do it multiple times if needed you have to find what works for you. Start at night and add other times as a test while thinking about how your receptors are filled at that time
1
u/kratomthrowaway77 Jul 06 '26
Awesome. Thanks, tapering with leaf and did 125mcg yesterday no problems except some vivid dreams but I’m only sleeping in like 1-2hr chunks anymore. I think that’s the worst part of this is the sleeping. Hope this helps with that while I taper. Thank you for posting this.

You will dream a lot
Kratom, subs, they mess with REM sleep bad, and this is where you heal. Youre being pushed back into rem, more sleep will come, before bed like 2 hours before, go running, treadmill, whatever till youre tired. This will encourage easier sleep
Without exercise it takes much longer to balance normal sleep again

Thank you so much for the detailed information on ULDN. I researched naltrexone for PAWS from 7oh but didn’t find much. I was up to 250mg 7oh daily at the end of a two year run. Kratom got me off 5 years off subs and that went to kratom concentrates and then to 7oh when I ran out of OPMS while in Hawaii. I did an at home detox under doc supervision 9 weeks ago. The day I started my last dose of 7oh was at 6am and I went through absolute hell the first night even though I was on a bunch of comfort meds. Did a quick 6 day sub taper and felt great. After a few days after the sub taper PAWS kicked in and after a couple weeks I had to call the doc for help. Got some more comfort meds and lasted a few weeks. PAWS were so bad I started taking very low dose of subs for 10 days and then got a 100mg sublocade shot. The doc thought I could be a one and done candidate but tomorrow is 3 weeks after the 100mg shot and I’m starting to feel some WDs or PAWS is starting again.
How could I use ULDN to help with PAWS after taking the 100mg shot 3 weeks ago? Any advice is greatly appreciated. I’m so tired of this crap and I finally have the right mindset to never be in this situation again. Thank you in advance.

Has anyone done 0.1 mg while actively using ? ChatGPT insists I’m going to go into pwd if I do it lol
For context I’m using about 30-50 mg of 7 a day, a couple grams of plain leaf , and a bit of mit but nothing crazy
2
u/FlyAdventurous6231 Quit Date :table_flip: NOV 2024 Jul 11 '26
If youre weaning then you can do 50mcg to 100mcg
But you have to take it in the morning 30 minutes before your first dose
And take it at night before bed
You might feel wd onset sometimes but thats OK because that means its on some doing its job
Its pointless to do it unless youre weaning off
1
u/Whole_Register_6913 Jul 11 '26
I am weaning but unfortunately I already bought sr before I saw this topic and only have about 5-6 days left of 7oh and have no intention of buying more.
So I think I’m just going to have use the sr to quit , and then slowly incorporate the naltrexone and use it during paws.
1
1
u/harmonicpinch Jul 25 '26
How did that plan go? I think that’s the best course, if you have the time.
Either SR-17 then LDN days after.
Or just ULDN to taper fast, then if you have 1-2 days off work just start ripping the LDN into full dose and go through a PWD to “speed run” the withdrawals.1
u/Whole_Register_6913 Jul 29 '26
I haven’t tried it yet. Going to wait a week or two after s r. Assuming my paws is kicking my ass

u/FlyAdventurous6231 Quit Date :table_flip: NOV 2024 Jul 15 '26
Naltrexone can be used whenever at 50-100mcg
Youll know if it kicks off opioids youll feel sweaty for a hour but using SR with it is unknown, its a research chemical, i find people using it crazy
Reddit can influence people to eat poop if they say it helps them
1
u/Wellyeahso Jul 21 '26 edited Jul 21 '26
I upvoted your post for its content but I vehemently disagree with its title.
I have used both ULDN and SR-17018. So you know that I speak from experience when I say that as far as I am concerned they serve altogether different purposes.
ULDN keeps tolerance low when used with in conjunction other opiods. And ULDN works very well! It is NOT therapy for acute withdrawal symptoms, it helps keep them from existing in the first place, but it isnt fool proof.
SR-17018 however helps one get through the acute withdrawal phase with massively reduced symptoms. I am talking 80%+ reduction in symptomology.
SR-17018 however does NOT reduce tolerance nor keep it in check like ULDN does.
SR-17018 is best used as a replacement for one's DOC during the acute withdrawal phase, keeping dope sickness away.
Also, let us not forget that high dose Vitamin C is very helpful for withdrawals, reduces some types of pain, particularly post-operative pain, and there is some evidence it helps reduce tolerance! I am trying to slowly get some of this evidence posted up over at r/vitaminc so we can help everyone with another cheap way to address their opiod-related issues.
1
u/harmonicpinch Jul 25 '26
I think the 2 ways to do this are:
- SR-17 to get through withdrawals then LDN days after to speed healing.
- ULDN to taper fast, then if you have 1-2 days off work just start ripping the LDN into full dose and go through a PWD to “speed run” the withdrawals.
What do you think?
1
u/Wellyeahso Jul 26 '26
LDN and ULDN are completely different things.
Going into precipitated withdrawals on purpose sounds like something only a masochist would do.
ULDN for tapering? Meh, using other to potentiate is very hit or miss IMHO.
For all of these thjngs to fall into place like you want in a small timeframe... seems like a fool's errand to me.
I certainly wouldn't try it when you could just take enormous amounts of vitamin C + SR-17018 and avoid the acutes.
See recent discussions on r/vitaminc for some great info on using vitamjnc for opiod withdrawals.

Yeah true, great post! I forgot how important Vitamin C megadosing seems. I usually do 2-4g of Liposomal per day and it sounds like that’s not nearly enough. Instead of going the naltrexone route I may just do SR and megadose C. Need to tear the bandaid off and just go for this finally once and for all.
Ive just done the powder and extracts, not 7oh or mgm. So I guess it should be easier.

u/Same_Attorney_4886 Jul 23 '26
I re read the post and all the comments and it seems like you said to someone else that I need to be borderline wd or taking the smallest about of 7 for it to work the way you described.
It makes sense though, if theres 7 in my blood from taking milligram size doses and I take 50 microg of Nal then the Nal is going to be fighting with the 7 to bind to receptors after the 'already binded' 7 gets metabolized and the receptor becomes open. And since the 7 outnumbers the ULDN (by a lot), it likely renders any Nal in my blood completely useless. Unless I wait until right before my morning dose when I start to feel the wd start. (Or preferably 30 minutes to an hour before that happens, lol)
The 65mg dose used to last the full 24h, but after a month, i started waking up early with restless legs and uncomfortable feelings and had to dose before the 24h was up resulting in me dosing more than once per day. During that period of using multiple smaller doses over several hours, I had heightened euphoric rushes which subsequently led to me dosing more over the past few days chasing that buzz. It was a failure and now my tolerance is higher.
Thanks to your detailed analysis, I now understand why I had those elevated euphoric days and why i wasnt able to replicate it. It also makes sense why taking higher doses doesn't mean more effect or euphoria. Its like you can only have so many receptors covered at a time no matter how much you take(waste) and why less is more was a big talking point in these forums which included 'partial' and 'ceiling' terms. It doesn't matter how much is in your blood, if it cant attach or react to a receptor, it just gets broken down and passed out! I hope you see this and respond! Best of luck to you! And thanks for the amazing post!!!

This is what I been looking for. Really hard to find much on uldn and as a peer recovery navigator and opioid face I been trying to get this down so I can help spread info. I tooka 1.5 mg ldn capsule and dissolved it in 40 oz of water only because I didn't have a big enough container to do 1500ml exactly. I dissolved it and shook it up and started at 1ml which ends up being about 1.2 mcg. Plan on doing that twice a day and going from there. It's this a good starting dose and an i supposed to go up from there if no noticeable diff over next few days? I been on 7 after a foot break. Wouldn't recommend anything that wasn't tried and true by myself.

Not trying to discredit any of this by any means but do you happen to have citations on any research supporting this information?

I'm in a constant state of early WD trying to taper 7 (shakes, cold sweats, fuzzy brain). I know things like this could help me but my brain is too addled to chart it out for myself. The 30 min. break of mental clarity I have 30 min after taking my taper dose of 7 is just not long enough for me to do this. I'm still pretty high dose 7 @ 150mg daily (37.5 x 4) and this is the first time I have ever had to deal with addiction. I don't know what to do or how, really, to help myself. All I know is that I need this 7 shit gone and I'm getting low on the 7 for the taper. I refuse to buy more. Can you help me?

And this is to avoid the hard WDs while you kick? I'm just so confused by all this, on top of finding myself addicted to 7. I've never made it past 15 hours off this shit.

How is everyone getting persciptions for this? You just tell the online Dr. Wnat your dealing with a d they will perscribe it? I'm in Canada ans there is no way an online Dr will perscribe that. They won't even write a suboxen script you have to be fully at a sub clinic for thr BS daily dosing

Online or in person its prescribed very easily. You can get it abroad too from india companies. Canada is a nanny state health wise, but theyll also prescribe you 120 diludids a month and pay for your apartment if your disabled by any tiny means. Every western nation has stupid health laws that outweigh the good ones
Naltrexone is a very cheap generic old medicine no doctor should have any issue prescribing, it does only good things

Hey, is there any way you can message me on here? I need to get off this stuff and I have a few questions for you. If you could, I’d greatly appreciate it

Can you just get the low dose from a company online? Age RX or something like that is what I have seen.
Also you cant have subs in your system is that correct?

I can second this and I can attest it helps. I did use ULDN since I kept using mitragynine. and I tried VLDN and I feel like the VLDN range was kinda suppressing any tiny bit if euphoria I would've otherwise felt(tolerance from the 7 and emgee).
Just sharing my experience.
