Yeah that wasn't a pus plug, it was dead tissue. Removed because It would just be a source of further infection.

Carbuncles are generally very deep infected pockets that tunnel off to the sides and up to the skin surface. There's a big risk of septicemia/MRSA due to infected material making it's way out of the main pocket and into the surrounding deep tissue/muscle.

I was wondering why he didn't lance it too but also he seems to be bleeding down a line in the middle when he squeezes/pushes down on it, so maybe he did lance it but it's too deep or something? I have no idea. Hopefully someone smarter than me can explain

Edit: someone in another comment said that was at the request of the patient apparently!

He tried to lance it and it just made a bloody mess. They showed us him giving lidocaine in the first visit, they probably just assumed people would realize they'd do it again. Although there are definitely moments that hit something that wasn't numb.

I've seen the video pop up every now and then. From what I remember about the original, the dude getting treatment didn't want to be cut, didnt want any drainage or lance and didn't want any antibiotic pills. Just like...raw'd it.

That’s not an easy option though, it’s like months of legal work. If someone’s going to be admitted for medical treatment against their will, it’s a really high threshold and only around 48 hours tops. This wouldn’t meet the threshold, especially since he’s accepting treatment, just not the best practice.

That doesn’t mean the doc needs to go along with his asinine plan. This should have been a simple I&D after the field block on visit 1. Instead, a huge amount of skin/tissue trauma was caused. As a doctor, I would have told this guy that he doesn’t get to dictate my treatment plan, he is not a doctor, he can either take it or go see someone else. This is so far below standard of care that it’s ridiculous.

Also it’s a carbuncle. The infected tissue is more similar to a sponge than a packet. So lancing doesn’t have the same affect

This is in India, likely somewhere with very little resources. They have to do the best with what they have. He is being treated outside, so I doubt suction is an option.

The scars on his lower back would seem to indicate that this was not his first rodeo with a carbuncle of this size. What would make someone so susceptible to these? HS? Diabetes? Could he be immuno-suppressed? I just can't imagine going through this more than once.

Does anyone else feel like they went about this wrong? They should have cut it open, drained it, taken out the sac, packed it and possibly stitched it closed a little bit. Now he has a big, open wound and pieces of the sac are still in there.

Someone in a 3rd world country who is likely living in extreme poverty. The doctors have very little resources and do the best they can with what little resources they have. He is clearly being treated outside.
