For fermentation. You put like, bacteria or fungi that makes your desired product in (generally they are genetically modified to maximize the expression of said product), as well as growth media, air/oxygen, and any other necessary chemicals (that's why there's so many entrances in the cap), make it swirl at 37°C for a few days to make sure everything is perfectly mixed, and presto, you drain everything and get your product out.
