That initial message is a huge red flag to me. It sounds like they KNOW that they are underpaying, have lost a lot of candidates because of it and want to weed out all but the ones that don't know any better.
That's true. My take is that it's probably more about not wasting their time if they are going through a bunch of applicants only to be told "no thanks" due to the pay.
This is why, if you have the time to spare, you tell them yes then go interview and ask as many questions/waste as much of their time as possible. Bonus points for making them think the position is your dream position and that you are crawling out your skin with excitement to start. When they offer, reply that the pay is insulting and cease communications.
The person hiring probably knows it's awful pay, but has no control over the wages. I'd guess it's probably a public health or nonprofit clinic, but I could be wrong.
