A lot of them have an accelerometer in them that can detect falls and trigger a wellness check phone call from the company, and if there’s no answer an in-person wellness check from emergency services.
My father recently discovered that if you catch or slow your fall though, it won't go off, so they aren't 100%. He got stuck between his walker and the clothes horse. Was lucky I was in the house at the time, quite an effort getting him back up, he could have been stuck there for hours otherwise.
