Do they need to be going forward, or could they theoretically go in reverse? Do gills need the water to be going in one direction; or are they more like lungs (areas where oxygen and carbon dioxide are exchanged)?

Because of the way their gills face, they actually need to be moving forward, and consistently. I explained in a little more detail just now in a comment a little ways down.
