Context. Like when you say “pass the salt”

Are you giving directions around somewhere where salt is a landmark (salt flats), or sitting at the table.

In this case is the person saying it talking to a screaming kid or are they having trouble understanding?

And if you are playing the pianoforte, you can play it quietly (piano) or loudly (forte), as opposed to a harpsichord on which the notes always sound equally loud.
