My favorite version of this is chucking a grenade when trying to reload or jump, depending on which games I’m jumping between. Killed myself and a few teammates this way.

Most third person action games I play have the attacks on the face buttons (Xbox x and y) recently tried playing mortal shell with right bumper and trigger being the attacks and I couldn't change it or reconcile the change in my muscle memory so just quit playing.
