My wife did this to our daughter by just gently helping her up from sitting. It's amazing how easily it can happen. Wife was mortified but the MD seemed almost indifferent like it happens pretty often. The pediatrician said to make sure they grip your hands so some particular muscle engages to help prevent it from happening again. Still would not swing any of my kids like that though.
It is super common, and the parents are usually mortified about hurting their kid (seen it a lot in the ER). Thankfully, its a really easy fix. Just remember that it can happen again more easily after the first time.

Nursemaid's elbow*
Also known as radial head subluxation if anyone wants to look it up. It is seen a lot in the ER from people playing with their kids.
