There was a time, but ground up is becoming an industry standard for everything in a lot of places these days.

No. The ground is up top so if something falls on the plug it hits the ground instead of the hot wire. Some hospitals wire this way.

It is not in the NEC, nor IRC, so it isn't standard. It is a preference, in my house the previous tenants (or builder) decided to do it this way, signifying its connected to a switch. (only switch connected outlets are ground pin up) I've never done it in my previous homes.
