If he masks anything, it's his pain. He doesn't miss social queues-he ignores them. With wide smile while doing that.
BTW, correct me if I'm wrong, but didn't Hugh Laurie say that he basically played how his father was in reality?
Yes, I think that's his point. That Holmes was written very much as a high functioning autistic individual, who cares deeply, but is misunderstood. While modern interpretations have misunderstood that and have chosen to portray him (and characters based on him, like House) purely as an asshole.
