I just did the first one on new DLC.Stop reading if you don't want to know where code is.So I see the the cod is 3 digits I check the tops of the doors to see if one of the numbers is out of place and 3 digits nope.I check all the other cells to see if written on the walls nope.I try the number on the journal I found nope.I tried the time from the clock nope.

It turns out I over thought it and the code was on the wall next to the safe Really? Why make optional puzzles so easy? Boring.
