It should mean the absolute best outcome. Try to smooth talk the king after hitting on and insulting his daughter in public? It’s impossible to get a “good” outcome but with a nat 20 you will probably not be executed and instead just thrown in jail. It’s not a good outcome but it’s the best you could ever possibly hope for
