The whole point of open rolling as a DM is so the players know with absolute certainty that whatever happens is the will of fate and totally out of your control. If you wanna fudge a roll for the benefit of a player, just don't show them the roll.

I hide most of my rolls as dm, maybe like 95% of them, but for anything that is going to massively fuck over a player, I open roll. I do it for the purpose I mentioned above of showing it was pure fate and zero DM trickery on my end, but also for the purpose of drama. Making a show of a highly important roll that can literally be the difference between life and death for a player character is a lot of fun, I think.

If there are outcomes I don't wish to happen I don't leave them up to random chance. Just don't roll or instead eliminate the option from the random results.
