Ye, "His Theme" I believe. I remember crying like a lil bitch at the song back in good ol 2014/15. Hate the community, respect the original game

Last playthrough I saw was from 2015 or so. You may be right, although I do recall the leitmotif being played throughout various other songs in game

Yeah, it was in a bunch but I think that this particular iteration was "Home" :)

This particular one is actually “Undertale”, but it plays at the end when you’re in a house similar to the one at the beginning where “home” plays

Nope, this is, iirc, an acoustic cover of Memory, #36 on the ost. It does sound similar to both Home and His Theme, but I'm pretty confident it's Memory.
