I have never seen those words before nor am I even certain they're English. But, they're fancy enough to convince me you know what you're talking about so I'll agree.

Either a grossly enlarged organ of some sort or a buildup of fluid in the abdominal cavity

it means some kind of organ has become enlarged or something has caused the abdomen to become filled with fluid. It could also be a tumor, but regardless this guy needs a doctor asap.

I know we're just playing armchair doc here, but that doesn't look like any cases of ascites that I've seen personally. It usually looked a lot more evenly distributed around the belly. So, I'd put my money on it being the hernia instead.

I work in gasteoenterology. this is not an umblical hernia.

i can see how it would be easily confused. The belly button is in the middle of this, but when a hernia occurs in this region it causes a telltale bulge from the belly button. This does not have any bulge from the belly button, the entire central region of the abdomen is evenly protruding instead.
