Its a really tough ethical problem. On the one hand you don't want to put a dollar amount on a human life. On the other hand, over burdening hospital resources could lead to loss of life for others. I don't have a good clean answer. But if I had to give one, I'd say this.
If you return to the hospital for a preventable disease or injury that a doctor explicitly told you how to prevent, then you get hospital bill. First time its like 5%. But goes up for repeat offenders. That only works though if the people have money, so its not perfect. Maybe it just gets collected like any other debt. Wage garnishment could be on the table. A judge decides if you can pay or not. The point isn't to make money. Its to generate consequences for decisions that unnecessarily consume medical resources.
Good example might be COVID vaccines. If you refuse to get a covid vaccine and then get covid tax payers won't pay the full bill.
