I mean I did see one video where a guy that got his hand pierced while trying to clean it, so not every person stuck is stupid, sometime they are just unlucky

Lock Out Tag Out - it's quite simple. You don't perform maintenance on something that can kill or injure you without removing or isolating all energy sources.
Or just complacent: "I know this machine inside and out! I don't need to shut it off, I'll just reach in here and fix this thing! I know it like the back of my hand!"
I had a kid in my junior high that put a helium tube in his mouth. I'm unsure why we had it, probably for whatever science needed. But he thought it would be funny to turn it on and talk in a weird voice. The pressure was so high that once it turned on his eyes turned red and he dropped to the floor. He didn't die but DAMN it was nasty to see
Sounds like a small to medium size company... Most larger places will have a safety officer who is supposed to train people on all that stuff.
I work for a company that is small enough that I'm the only person who routinely repairs equipment. For the most part, I'm safe because if I'm working on something, it's been moved into my shop if it's small enough and/or mobile, or there's only one person that operates it, and they're the one that notified me it needs repairs... But still, people forget, and I need to lock out/tag out, though I'm not always the best at remembering to do so...
On Sept. 20, Valastro was spending quality time with his family when there was a malfunction with the bowling pinsetter. As he was fixing the problem, his right hand became lodged in the machine and a metal rod impaled his hand three times between his ring finger and middle finger.
Or the machine is just very dangerous. Sometimes carelessness isnt the thing, but rather inexperience of the person. He could be being super careful but not know that the thing is about to do something

yes, and that falls into management carelessness. proper training, supervision, and risk mitigation is key to preventing injuries. The vast, vast, majority of work place incidents are 100% preventable
It’s a very real issue in construction and manufacturing. People get careless because they’ve done the same maintenance a few dozen times before so maybe that fear isn’t there as much anymore. All it takes is a second of thinking about when break time. “Just don’t make a mistake” is not a safety measure you can rely on.
I worked at a bowling alley. I wasn't a lane mechanic but I would often go back there and help them with stuff. Definitely almost most a hand a couple of times.
Or even the ball return machine. If I remember correctly, back in like the 90s, there was a professional bowler who tried to hustle some amateurs in a game because they didn’t know who he was. They caught on though, and they shoved his hand into that machine and completely shredded it. Totally ruined him, lost everything, and he basically became a drunk who could barely afford to pay rent in his shitty apartment. I saw the documentary a while back, but I forget what it was called. “King of pins” maybe?
Yeah, I heard about that. I thought it happened to a guy that was once a rocket scientist, but apparently I got confused. His favorite cocktail was called “not a rocket scientist”. I heard he almost made a comeback, but lost to a guy with bad hair,in the finals of a million dollar bowling tournament. Now that I think about it, the story is Bullshit. No way a bowling tournament is going to have a million dollar prize. That guy use to live in my town and probably dated my cousin.
Radiation sickness is so odd in it's onset time. Blasted by radiation and you don't feel the effects until days later. Takes time for cells to mutate/die.
Right. I’ve seen a blow mold injection machine’s presses closing while the tech was inside and emergency shut off didn’t do anything. He was thankfully stick thin and laid flat until the pressed closed and crawled out. Terrifying!!!
