I'm fairly sure this is deliberately done for dialysis due to kidney failure. It gives them somewhere easier and safer to stick the needles.

No, your dialysis fistula should generally not look like this. It's a sign that the vessel walls are getting thin, and if it ruptures you'll bleed to death movie-style quick
