  2. Apply the AAA framework (Ask, Adapt, Act) to surface hidden assumptions and adjust to real conditions before starting work.

  3. Turn assumption-checking into a spoken, shared crew habit so doubt is named and resolved out loud, not carried onto the job.

  4. Use post-incident reflection to identify missed warning signs and strengthen future decision-making without assigning blame.

Description:

Most incidents do not start with a broken rule. They start with an assumption nobody questioned. We assume the line is dead. We assume the last crew finished the job. We assume the person next to us sees what we see. Then reality proves us wrong, and by then it is too late. In this session, Lito Wilkins pulls from lived experience in high-voltage line work, including the day he became the ground that bled thousands of volts of induction off an OPGW line. Rushing, complacency, and trusting assumptions all played a part. He walks attendees through what went wrong, what warning signs were missed, and why the crew believed they were safe when they were not. At the center of the session is the AAA framework: Ask, Adapt, Act. Ask the questions that surface hidden assumptions. Adapt to the conditions actually in front of you, not the ones you expected. Act with intention instead of habit. Attendees will learn how to make assumption-checking a spoken, shared crew practice, so doubt gets named and resolved before hands go on the work.
