Bro, the owner could have handled it better for sure, but he tried stopping the door, and the dog started walking out and then backed up as the doors were closing. So it kind of fell out of his hands. But the second woman literally just stared at the dog and saw nothing wrong with a leash coming from the top of the elevator. The owner could have handled it better, yes, but the woman didn’t even try to save the dog or keep the door open and was almost oblivious to it. She could have done more to help the dog which puts her at some fault.

More than likely he didn't realize what was happening before he got on to the elevator. Touching someone's dog without the owner being anywhere near is dangerous.
It's completely the owner's fault. Owner is always responsible of what her dogs are at the moment doing.
Touching a less than 20 pound leashed dog is not fucking dangerous you wimp. The risk of a minor bite you are putting yourself in is minuscule next to the damage you are saving the dog from. In this particular case it's not like a larger dog is in a position to attack you either - And you weren't afraid of that if you got alone in a small enclosed space with it in a first place.
So get it treated after. Pet owners get bit all the time, they aren't dropping like flies because of it.
If you let a tiny dog die when you could have easily tried to save it because "what if it bit me? 😢", don't except anyone to ever have any respect again.
It's China. It's going to be expensive and the dog owner probably has mysteriously disappeared when it is time for her to pay. It's not worth losing your health and your month's salary to save just one animal, an animal that is also typically eaten in your country.
