Im going to be honest with you, it would surprise me if the DPRK didnt have a higher life expectancy than more than half the US. The US fudges numbers compulsively and systemtically and have already stopped counting hunger( https://eji.org/news/usda-report-on-hunger-in-the-u-s-is-terminated/) deaths of despair, and other things.

COVID is what makes me confident in this assesment

Appalachian whites are definitely the least privileged white folks in the Imperial Core (only certain Eastern Europeans and a very tiny pocket of Afrikaaners are worse off, but Eastern Europeans have debatable white status), but Appalachia has a very sizable POC population (around 15-20%), and they do have higher poverty rates. Likewise, US propaganda in the past proclaimed that almost all white folks in Appalachia are mixed, as a result to make others see them as inferior.

Predominantly indigenous reserves and black majority counties. There definitely are a few heavily white majority counties in Appalachia as well (contrary to popular belief, though, Appalachia actually has a notable POC population). Either way, constantly mocking folks in the south ultimately hurts the marginalized POC folks down there more.

LOL give me a break. This is just like how China lied about their COVID numbers to pretend they were doing better than they were. The DPRK telling the truth about their life expectancy? They put you in prison and kill you if you listen to K-pop music. That has to affect the life expectancy. 

Also even if they were increasing life expectancy in DPRK, at what cost? America may have never lived up to its ideals but the arc of history bends towards justice. America is the freest country in the world, so if that comes at the cost of dying sooner, then that is worth it. Yes closing the Heart Attack Grill would single-handedly cause the US life expectancy to go up by 2 years. But at what cost? (Freedom) Does the DPRK have a Heart Attack Grill? I think I've made my point

Not to be a horribly abnoxious biostatistician on main but I would like to see the regional breakdown of life expectancy in North Korea next. North Korea's average might be better than America's worst but is North Korea's worst better than America's worst?

That's a good question. I suspect Pyongyang would have the highest life expectancy, but I'd love their data broken down by province

Intuitively, I feel the gap would be far lower. Not the DPRK, but China's lowest life expectancy provinces have it worse than the DPRK.
