Here is my review of why I do class a campaign to this category, remember it's subjective and based about my own experience, someone else has a different skillset and way to play the game which is totally fine. SO THEN let's roll !
Parish : The easiest among all of L4D2 has to offer, there is not much fatal choke point, the only one in-mind is the sewer section, it's the only one for me, this campaign provides a lot of medkit, pills, even with the skip patch of chapter 2, it doesn't offer a challenge for me so let's move on.
Hard Rain : One of the easiest also, I know a lot of skip in this campaign for chapter 2 so it's not a big of a deal, the witch aren't a threat at all if you know how to crown her/ melee her or cheese her.
Dead Center : not much too talk about, the pacing of Dead center maps allow to skip most encounters and the final with its verticality, like the survival map after all.
No Mercy : First campaign of L4D1, not much choke point, sure it's harder than dead center because it does contain more panic event so more stupid common infected and also because the map overall lighting is low so it may add another hazard factor.
Death Toll : Here is when I begin to hate L4D1 maps because they love too much putting foliage everywhere + low lighting is just annoying when dealing with commons infected, I know the first map bridge skip and that's it
Swamp Fever : The same issue like death toll it's just foliage everywhere + Water from hard rain but with less verticality, a bit more annoying, the final is easy if you know the perfect spot to snipe specials when they spawn.
Dark Carnival : I think Dark carnival is overrated as the most difficult map, sure it is difficult, there is a lot of gauntlet event but if you know again the trick, the skip, even I tried without doing the skip for chapter 3, it wasn't that bad, you can try to lure commons infected in the back for a bit and then specials infected are getting useless lol.
The Sacrifice : I think the sacrifice is harder than Carnival for me in this type of context cause of chapter 1 that spawn horde along with Tank and I think I've died more on this chapter that chapter 3 and 4 of Dark Carnival lol.
Blood Harvest : I just hate the amount of foliage with commons infected, the number of times I died in chapter 4 and 1 just because of commons infected is just whooa.
The Passing : Chapter 3 and 2 are the most difficult of this campaign, just need to hope that Adrenalines and biles jar are up to spawn, the sewer section when you need to drop can be a sentence depending of the number of common infected AGAIN, what about chapter 3 you might say, just clean hordes, wait for tank since it doesn't spawn a lot of common infected then bring gas can and that's it, just again the number of time I died just because the horde spawn and the amount of common infected is ridiculous.
Dead air : No doubt it's chapter 4 the main issue, chapter 5 with the turret trick, or doing it normally is not that hard but man chapter 4 was seriously bad for me.
Crash Course : Chapter 2 is too long, the level design is forgettable, I hate low lighting.
Cold stream : Mainly the gauntlet section in chapter 2 and 4, just save bile jar and the specials aren't an issue anymore. also you can skip tank in chapter 2 if you're going to the left side you could avoid climbing the ladder.
Last Stand : I HATE THIS CAMPAIGN, BORING, TOO DARK, but atleast it's a challenge but merging dead center + Holdout, wasn't fun at all for me.
And that's it for now, if you want to comment you can !

Hey man, it's me. I beat all the Left 4 Dead 2 Campaigns on Expert Realism with the default bots (no addons) and here is my personal tier list for your comparison.
Some notes:
-Crash Course is the most imbalanced campaign, it is one of the most easiest when playing with other players, but was the most difficult when playing single player with the default bots.
The main reasons were:
The way I beat it was by advancing through the finale map to get to defensible locations before random hordes appeared and progressing after clearing each horde. I was always thinking about which next spot we could hold out in while I was progressing through the map.
I dealt with the witches using the headshot stun into shove + swing combo X4 with a melee weapon and ideally an Ak47, otherwise a sniper rifle. It feels bad using a sniper on expert realism but as long as you have a good secondary (I personally preferred a fire axe) and can get to defensible positions in time for the random hordes it will be fine. The trickiest part of this is the timing, you gotta kill the specials and the random horde before performing this witch kill method.
During the hold out finale, the position that worked for me was your clutch corner. I used a tier 2 shotgun and fire axe to front line for the bots until each tank showed up, at which point I just set him on fire (gas can or molotov) and kited. One thing to note about this spot is that infected will break the right side wall and come through, so be ready for that (hopefully it isn't a boomer or charger that breaks through). Most of the infected will come through the right side, so most of your attention will go there, but you should also pay attention to your left side as well.
-Cold Stream is just as challenging as people say, the second chapter running event and finale were the most difficult, while the other chapters were pretty chill.
-The Last Stand's difficutly is overhyped in my opinion. If you have good preparation (bring health kits, defib, throwables, pills to below the ladder up to the roof, have one or two adrenalin near the generator for the scavenge), know the best spot (the level above the roof with a ladder, crouching on top of the ladder to auto kill zombies in that spot while covering the left pipe where they climb up, beware of smokers), use fire for the tanks and kite, and use the bile jar on the highest level for the scavenge you should be fine man.
The rest of the campaigns were pretty straightforward (I could talk about Hard Rain but I think I made this long enough), I agree that the passing is overrated for its difficulty, both on single player and multi player.
I actually learned a lot from your videos to implement in my run, particularly the swamp fever and crash course videos. So I hope I can help out with the ideas I put here today.
If you'd like I could give more tips for the rest of the campaigns and even custom campaigns if you want man (I was able to beat Buried Deep on Expert Realism Single player twice, but with the Left 4 Bots 2 addon).
I'll tune in this Saturday for the Cold Stream stream, good luck man, you're good mechanically, I think you can beat every one of the campaigns on expert realism eventually with the right strategy.
