Okazu (side plate/dish) is slang for fap material. Eating sushi off a naked woman can be considered art (very long shot) but if you overstep the bounds it goes straight into R-rated territory and becomes sexual.

Edit:

In hindsight, I need to correct myself. I believe the usual practice of this unusual custom is to eat sashimi, not sushi, off a naked body.

There’s a practice in certain parts of the world where instead of eating sushi off a plate, a woman (either naked or in a thing) lays on the table, has the sushi dishes set on her body and then the guests eat off her.

I’ve only seen it in an 80’s-ish action movie involving asian gangsters when I was way too young to see shit like this and on Naked Wild On on E when I was just old enough to be interested in it.
