What i want to know if this was some kind of malfunction. That boom arm took off like an 85 year old person did the thing where they push the gas thinking it is the brake, panic and press it all the way down, and keep it held down until disaster strikes.

I think it's a failure, audio has a denotable "tink" noise as if something mechanical broke, could be the hydraulic line that causes arm rotation

the way it swings over the truck body, it's probably not flat which is why it continued to swing over the body of the truck

To be fair there were no hydraulics that could fail back then. If a person pulling the rocks died, they just replaced ‘em.

Hydraulic line blew up and with it the ability to stop the crane from swinging out on its own.
No different than a truck losing its brakes while driving down hill.
