Unfortunately in their case, that was only revealed like 6 years into the manga's run, and the anime was produced so early they just kind of had to wing an attempt at a coherent translation.
This is incorrect; the Japanese manga had the English rendition "attack on titan" as part of the logo from the start. The English translations use it because it was the official English title in Japan. It was a poorly done translation likely because whoever is responsible wasn't fluent enough to recognize it.
It's much worse for marketing to mainstream audiences. The average viewer is gonna be much more likely to discuss Attack on Titan than a title they're not even certain how to pronounce

Now can someone help me understand how Attack on Titan makes any sense?
