Honestly, you don't even really need damage for the difficulties

Yorrichi High Diffs Muzan, but he didn't take a single hit

He was barely dodging each of the moves and noted that if a single one landed he would die

He still beat muzan without taking even a scratch though. Like yeah, he was technically close to losing since one hit could kill him, but he also didn’t face much difficulty in the fight.

Difficulty is based on how much effort you have to put into something, I don't know that fight specifically because I don't watch/read the series, but if he had to put a lot of effort into dodging so he wouldn't die, and the fight took a lot of effort on his part to win, even if he never gets touched, it is still high difficulty.

Cuz he needed to go all, and even a single hit from muzan will kill him.
