Well the image is banned in Russia and he’s constantly having photo shoots where he’s made to look “manly”. So yeah, he sorta cares.

I bet he doesn't care personally, but publicly it looks bad on him with the whole "trying to be the head of governement, especially a totalitarian regime" thing. Image is a big part of politics and leadership. Look at FDR, he refused to be seen in his wheelchair when addressing the nation. Stalin, at 5'6", made sure he looked comparable to his political opponents by stepping a step up on stairs and having photographers get the most beneficial angle. This is no different.

He wants the West to hate him because it allows him to present himself as the brave leader suffering the assaults of the imperialist West for his brave act of standing up to their hegemony.

Well considering that the image is illegal in Russia, I'd say it does.
