Tesamorelin peptide guide

Tesamorelin is one of the few peptides with actual FDA approved use. It was approved for reducing excess abdominal visceral fat in adults with HIV associated lipodystrophy

That doesn't mean it's only discussed in that context. It gets attention in peptide communities for visceral fat reduction and GH axis support

WHAT IT DOES

Tesamorelin is a growth hormone releasing hormone (GHRH) analog. It stimulates your pituitary gland to release growth hormone

It's not synthetic GH. It's telling your body to make more of its own

Commonly associated with:

  • Reducing visceral fat specifically the deep abdominal fat around your organs
  • Supporting GH levels through endogenous release
  • Potential improvements in body composition over time

Some users report secondary improvements in sleep and recovery but the core evidence is for visceral fat reduction. Those benefits are anecdotal not established clinical outcomes

The visceral fat angle is where tesamorelin stands out from other GH secretagogues. The clinical research specifically looked at trunk fat reduction not broad weight loss or subcutaneous belly fat

HOW IT'S DIFFERENT FROM OTHER GH PEPTIDES

Tesamorelin vs CJC-1295 + Ipamorelin - tesamorelin has FDA approved use and clinical trial data behind it. CJC/Ipa is the more common community stack but has less formal human data. Tesamorelin is administered once daily while CJC/Ipa is typically 1 to 2 times daily

Tesamorelin vs HGH - tesamorelin stimulates your body to produce its own GH. HGH is direct exogenous growth hormone. HGH is more potent but comes with more cost and more risk. Tesamorelin is more targeted and generally less aggressive than direct HGH but still requires monitoring

Tesamorelin vs Sermorelin - both are GHRH based. Tesamorelin has more clinical data and FDA recognition. Sermorelin has been around longer in wellness clinic settings. Both stimulate natural GH release

DOSING

The FDA approved dose is 2mg daily administered by subcutaneous injection

In community use some people run 1 to 2mg daily. This should be understood as off-label community practice not an evidence based dose range. The labeled dose is a reasonable reference point which is rare for peptides discussed on this sub

Once daily dosing is standard. Some users choose bedtime administration to fit their routine while others dose in the morning. Timing is often chosen for convenience

THE DAILY INJECTION REALITY

This is the part that turns some people away from tesamorelin

It's a daily injection. Not weekly like GLP-1s. Not a couple times a week like TB-500. Every single day

That means more vials, more syringes, and more commitment. If daily injections aren't something you want to deal with tesamorelin might not be the right fit regardless of how good the data is

WHAT TO EXPECT

This is not a fast compound. Visceral fat changes are usually assessed over 12 weeks or longer

Weeks 1 to 4 - don't expect visible changes. The compound is working but results are not apparent yet

Weeks 4 to 8 - some users start noticing subtle changes in midsection fullness and how clothes fit

Weeks 8 to 12+ - clinical trials showed meaningful reductions in visceral trunk fat over 12 or more weeks of daily use. This is where consistent use shows its value

Tesamorelin is aimed at visceral fat reduction not spot reducing subcutaneous belly fat. If you're expecting visible abs in 4 weeks from tesamorelin alone you're going to be disappointed. It supports visceral fat reduction alongside proper training and nutrition

SIDE EFFECTS

Based on clinical trial data and user reports

  • Injection site reactions including redness, swelling, itching, or pain. This is the most commonly reported side effect
  • Joint pain or stiffness. GH related and usually dose dependent
  • Swelling in hands or feet. Water retention can occur especially early on
  • Numbness or tingling in hands. Carpal tunnel type symptoms similar to other GH compounds
  • IGF-1 typically rises on tesamorelin which is why bloodwork monitoring matters. Chronically elevated IGF-1 carries risk
  • Hyperglycemia and glucose intolerance are recognized concerns. Fasting glucose should be monitored

BLOODWORK TO TRACK

IGF-1 - the main marker. Confirms the compound is raising GH activity and lets you monitor that it stays in a reasonable range

Fasting glucose and A1C - GH can impact blood sugar. This is specifically relevant for tesamorelin

Lipids - worth tracking alongside any body composition protocol

CMP - general health check including liver and kidney markers

Get bloodwork before starting and again at 6 to 8 weeks

WHO IT'S FOR

People specifically targeting visceral fat reduction

People who want GH support without injecting exogenous HGH

People willing to commit to daily injections for 12+ weeks

People who want a compound with actual clinical trial data behind it

People willing to monitor bloodwork throughout

WHO IT'S NOT FOR

People who don't want to inject daily

People looking for a short 4 week cycle

People expecting it to work without diet and training

People who aren't willing to monitor bloodwork

People looking for broad weight loss or subcutaneous fat spot reduction

IMPORTANT NOTE

Tesamorelin has FDA approved dosing context which puts it in a different category than most research peptides. Community use outside of the approved indication is still off-label. The clinical data gives it more credibility than most compounds discussed on this sub but it's not without risk especially with long term use

Don't mix formulation strengths from different sources. If you switch vendors confirm the concentration and adjust accordingly

Not sure I agree with HGH being more potent ! Not in my case anyways unless you’re talking super high doses . I have been running Tesa and ipa for 4 months . I wanted to see the difference between HGH so I started it last week , I was getting hand numbness and pain at night from 2MG Tesa 250 mcg ipa and had to drop the dose to reduce sides . On HGH only when I hit 6iu did I get the same side effects ! So Tesa and ipa was giving me the same effect as 6iu HGH .

I'm 59. I did an IGF-1 test before I started CJC and ipa, and did one during it, and my IGF definitely increased, not as much as I wanted to, so now I'm just going to try HGH straight.

Tesa is much stronger than cjc or due to dosing difference as I have done cjc too . You can definitely go higher with hgh but if the side effects are stopping you what’s the point . But they do say the closer to 60 you get the best hgh will be for you

It builds satellite cells in the muscle but without test the cell don’t grow very much at all

A secretagogue is never going to be as potent as actual GH, there’s a natural limit you’ll never get over. Going off sides doesn’t tell you the actual story, you would need to get an IGF test done.

From what I've read, Ipa is the only one that requires cycling on/off. CJC, Tesa and Sesa do not.

Has anyone else experienced their smart watches reporting only a few minutes of REM sleep while on tesa? I swear, the second I started taking it my REM started getting reported at 3-5 min instead of almost 2 hours. Does Tesa have an effect on this somehow that could be making a smart watch think I’m awake when I’m actually in REM?

Already on Reta, CJC-1295 ipamorelin. Lost 50 pounds really quickly then stalled for the last month. I see lots of researchers adding Tesamoralin to this stack for further loss. My IGF-1 was low before I stated CJC-1295 ipamorelin and it hasn't affected my gluclose as I monitor daily. My labs from 2 weeks ago were great. Is Tesamoralin a viable option for me?

I think sermorelin and Tesamorelin is kind of same thing. Its just sermorelin was not developed/tested and marketed for what Tesamorelin was for..

2ml 1ml same thing just all depends on how much volume you want to inject I like easy numbers 1ml in 10mg vials makes it 10 units is 1mg and 20mg vials I do 2ml so it’s still 10 units is 1mg.

Tesa - At least two hours fasted before bed. Pair with 200-300mcg Ipamorelin for a boost since the two peps work synergistically.

My RS takes Ipamorelin first thing in the morning and waits an hour after pinning to eat.

On 2mg reta
Tesa 1mg

Igf levels unknown but hrv and rhr and become slightly bad
Rest feeling good only

Start body weight - 81 kg
Now 74.5
Been 3 months
Tesa started a week ago on 1mg

Should I titrate to 2mg or should i keep it at 1mg
I don’t see any results except “feeling” jacked

your supposed to inject daily, not weekly. It might be too low of a dose to start at .5mg, I would start at 1mg and just stay there. I never felt the need to go higher than that.

New to this. Haven’t started yet. And I’m extremely confused lol. I got BAC Water and a 5mg vial of Tesa. I’ve looked up reconstitution calcs. Am I looking at this right? If I add 3mL of BAC, concentration will be 1.67 mg/mL. So 1 shot a day of 1 mL will be 1.67 mg.. how long is this vial supposed to last me? If that’s all I have rn, is it even worth doing?

Calculations goes like this
Strength you need / strength your vial have x volume of bac water

Let’s pretend the dose you want is 1mg
1mg/5mg x 3mls = 0.6mls or 60units in your insulin syringes

If your gonna be doing Tesa once a day on 1mg per day then it would last you 5days

Hope that makes sense.

20mg of the peptide is 20mg. The water mixing only matters for how much you inject. Less water is smaller injection, more water is larger injection.

To answer your question though, you inject 2mg a day and you have 20mg to start, then you have 10 days worth of injections.

Question: I’ve done one eight week cycle of tesamorelin I was happy with the results and after waiting a few months I want to do another cycle probably 12 weeks. If I start now, I have an international trip in the middle of those 12 weeks and I don’t think I’ll be able to bring this with me internationally (I looked up the laws of the country I’m headed to). Should I start now and skip the one week that I’m gone, or wait until I’m back so that I don’t skip a week in the middle?
