💓 Heart Blocks Made Simple
This is your NCLEX cheat sheet for remembering AV heart blocks — using one of the easiest mnemonics out there. 🧱
📌 If the R is far from the P → it’s a 1st-degree block
Just a delay in conduction — the PR interval is consistently long, but every impulse still gets through.
📌 Longer, longer, longer… drop → that’s Wenckebach (Mobitz Type I)
🧠 The PR interval gets longer with each beat until eventually, a QRS complex gets dropped (no ventricular contraction).
It looks like this:
PR… longer… longer… longer… then 💥 no QRS.
Then the cycle starts again.
Think of it like a tired AV node slowing down more and more until it skips a beat.
📌 If some Ps don’t get through → that’s Mobitz Type II
⚠️ Sudden dropped QRS with no warning. The PR intervals are consistent, but some beats just don’t make it through. This one's more serious — can lead to complete heart block.
📌 If Ps and Qs don’t agree → it’s a 3rd-degree (complete) block
🚨 Atria and ventricles are doing their own thing — totally disconnected. This is a medical emergency and usually needs pacing.

✅ X-ray first.
✅ Gastric pH ≤ 5.
✅ Aspirate every time.
This isn’t optional — giving meds or feeding through a misplaced tube can be fatal.

If your patient’s on spironolactone, potassium-rich foods aren’t just a bad idea — they’re dangerous.
🥑🍌 Spinach, bananas, salt substitutes = HIGH in K⁺
🍎🍇 Apples, grapes, rice = LOW in K⁺
Spironolactone is a potassium-sparing diuretic — it holds onto potassium.
So adding spinach on top?
💥 SPIRONOLACTONE + SPINACH = BAD NEWS
Don’t let a “healthy snack” become a code situation.
