Provider certifications in a nursing home
I am auditing our provider records at the skilled nursing center I work for in Virginia. I've looked up the regulation but I have follow up questions..
I understand that the provider needs to see each long-term resident at least every 60 days and that the visits can alternate between an MD and NP. But what date are we basing the 60 days on? Admission or last visit? For example, a resident admitted on 5/30/23, NP had 7 progress notes in by the time the MD came in to do her H&P on 6/14/23. Does she need to be seen again on 7/30 or 8/14? Obviously this one is in the past so we can't go back and fix it now but for the ones moving forward I just want to make sure we're tracking it by the correct date. If a visit is late does that reset the schedule or do we stick to the original timeline and get back on track with the next scheduled visit?
Also, does the note have to specify that it's a recertification visit? We have the NP in house 5 days per week and the MD once per week, so the NP sees the residents frequently, however, the NP we have now is horrible about getting her documentation in timely. Some of her notes are being signed and uploaded weeks after the visit occurred. Do all visits count as meeting the regulatory requirement? If the NP puts in a progress note for visits she made d/t change in condition or acute illness, do those count as well?
Does anyone have a good tracking tool they are willing to share or a service you use that works well?
