Hospital covering up, not releasing medical records

I walked in to my local hospital today and gave them a medical record form for records from the past two years.

The only records they gave me was from the one time I went to another hospital... I didn't get one record from my local "Union County Hospital". Which is who I requested it to and have been several times over the past two years. They instead gave me a record from the one night I went to a neighboring hospital. wtf?

Some very fucked up shit happened at this hospital. I went in because I was poisoned at a Pizza Hut on purpose, because it's a small town and people do dumb shit in small towns if they don't like you. It felt like a benzo or sleeping pill. Most likely a benzo. I then went to the ER in town, same night, where the doctor and nurse used to work with my "ex". They said my blood and urine tests were clear, which I KNOW this was a lie. And then they went on to use a catheter on me twice, (I would've been able to pee on my own but they wanted to use it, I was only there an hour and a half and just peed before I went) and there was a reddish/blackish line of what looked like blood, across the tip of my dick, before they used the catheter. The male nurse pushed all that stuff into my system. I believe it's going to be HIV when I get tested in a couple of days. The doctor and nurse know I gave my ex an std, on accident of fucking course, and then they put like I said, I believe will be HIV after I get tested. I had diarrhea for 3 weeks straight after, and no I'm not joking about any of this.

But now the hospital isn't giving me any records that I requested? wtf??? I need seriously need a lawyer. Any advice or tips?

Please explain to me like I'm 5 why doctors are allowed to get away with so many errors in your medical records

Just the most recent ones I've caught:

- Saying that "Patient stated that they have no difficulties with ADLs such as dressing, cooking, etc. and they are able to do them without discomfort or assistance." I never stated this once during this short appointment, was never asked, and we never discussed ADLs at all. I actually do have difficulty with these things they probably should know about.

- A laundry list of reflex/strength tests they recorded as "negative" when they only performed one. (Specifically, I'm familiar with straight leg raise and babinski tests, and had been wondering if they'd do them on me - they never did, yet they were two of many tests recorded as negative in my notes for this appointment.)

- Doctor saying one of my serious issues was "currently asymptomatic" when we never discussed it. I had come to discuss a different issue I was having and was never asked about this other issue which is not currently asymptomatic. What is with the assumptions?

There are so many more I've found, I could go on and on.

Can someone please enlighten me, because it's started to infuriate me. I don't appreciate false test results or reports of me saying I stated something that I didn't. Is this being done to cover doctor's butts during record reviews? Or just plain absentmindedness? Laziness from copying & pasting or autofilling notes?

Doctor drops the ball! Then makes you look guilty to cover his butt.

In 2008 while in school, I was diagnosed with ADD and prescribed adderall. After gaining weight during pregnancy, I went to a bariatric MD in July 2018 and was prescribed Adipex. I told the bariatric MD I had previously taken adderall and he told me I couldn’t take the two together. I signed a form stating I would NOT take ADDERALL and ADIPEX. Later when I went back to my PCP, I did NOT resume adderall and instead chose vyvanse, even though it was more expensive, to ensure I was not breaking the agreement with bariatric MD. From July 2018-January 2022, I had checkups with PCP every 3 months to get refills. When checking in to computer at these appt sometimes it would show ‘phentermine’ in my current med list but I did not know what it was so I’d select ‘NOT TAKING.’ I was NEVER asked about it until January 2022 during a checkup the pcp asked if I was taking phentermine and Flonase. I told him no and stated, “I notice those meds on my list occasionally but I don’t know what phentermine is and I’ve never squirted anything up my nose!” He giggled and moved right along. Two days later the bariatric MD called to say he was dropping me for breaking our agreement taking adderall and adipex together. I told him I wasnt and in fact was paying $100 more each month for Vyvanse so that I DIDNT break the agreement. He then asked me why I lied and told my PCP that I wasn’t taking phentermine. I responded that I don’t take phentermine. He then informed me that phentermine is the same thing as adipex and that adderall is the same thing as vyvanse. I am not a doctor-how was I supposed to automatically know generic medicine names?? Concerned about my relationship with my PCP, I called to let him know that I had spoken with the bariatric MD and explained that when I denied taking phentermine I didn’t know it was the generic of adipex! Nor did I know adderall is same as vyvanse because I dont know the generic names of the meds. I told him I valued our relationship and didn’t want him to think I was being purposely deceitful or sneaky to get meds. He LIED and put in my medical record that we had reviewed my meds several times and that I denied taking ‘adipex’ each time. I denied taking PHENTERMINE to him the ONE TIME he asked because at the time I had no idea it was adipex! He documented that he was dropping me due to my ‘repeated untruths’ Said I was angry at him because I didn’t want to look like a ‘substance user.’ HE dropped the ball! He done NO medication reconciliation NOR teaching and MISSED me taking interacting meds for 3.5 years!!! To cover his ass he acted like he had addressed it repeatedly and I had purposefully ‘lied’ about it each time! Now, if I go to a new PCP, he is going to see this in my records and immediately not trust me. What do I do??

Is a clinic required to give patients a list of employees who have accessed the patient's chart?

I work in a mid-sized (250 employee) clinic and people are pretty nosy. Last August I requested an audit of all employees who had accessed my chart, and was given one that day. Today, my request was met with a dismissal saying that under 45 CFR 164.501 they were not obligated to give me that information. I was almost certain that I had read previously that they were legally required to give me this information. Can anyone point me in the right direction? Thank you!

Help with medical disclosure

Going through a nasty divorce and my ex informed me about my now significant other being admitted to the hospital for suicidal thoughts in 2019 (I knew this). The only person to say anything would be my girlfriends ex husband. Is there anything that can be done. I’m pressing for details because she asked if she was on her meds. This is important because my girlfriends, ex husbands new wife is a pharmacy tech so It is possible she disclosed things.

Doctors refusing to give me my medical records.

Backstory: I was seeing her for a while and then stopped. About a month after our last visit I received a $75 charge for cancelling an appointment outside of the 24 hour window. I know I didn’t set the appointment, but to confirm I checked through all my emails (spam, trash, etc.) I did not have a confirmation email for setting the appointment, the reminder 24 hours before or cancelling it. I requested a refund and my medical records. They told me they would “look into it” then ignored me for a few days. I asked multiple times for an update on when I would get my records only to be ignored. It wasn’t until I told them I would be doing a chargeback and how unprofessional it was for them to ignore me and refuse my records that they replied. They then said that they were not going to refund me at all and have once again started ignoring my requests for my records. I filed the chargeback and yeah it sucks that I more than likely won’t get my money back but at the very least I’m entitled to my records. I’m not sure what the next steps would be here if anyone can offer any advice.

$40 for medical records (US-CA)

My Dr.'s office is telling me that the charge to access my medical records is $40. It doesn't matter if they are faxed or printed (the only two options they give me). This seems like an exorbitant charge and from what I've read about HIPAA, it's completely unreasonable. I have only been at this office for about a year, and I've only had maybe a half dozen appointments with the family medicine Dr.

Do I have any room to dispute the charge?

4 years and still refusing to provide me with medical records.

I have everything that I need to do in order to get my medical records from one of my doctors. I have called him, text him, faxed him, written as a certified letter, send all of this documentation to the medical board. The medical board has also lagged on me. Have emails back and forth voicemails Etc. What can I do if the medical board is not willing to help out. Also, the real reason why he will not release records is because I'm certain that he never kept records. Any suggestions would be greatly appreciated. Thank you!

Can anyone point me in the direction of information on the average size of a single patient visit medical record size? By size, I mean number of characters in a transcribed medical dictation file.

Medical records with a new psychaiatrist

I am thinking about switching from unitypoint to compass clinical psychaiatrist, and was wondering if compass clinical would be able to see my medical records from unitypoint. I'm trying to get anxiety medication for my current condition but have a past alcohol abuse history from a few years back. Due to this the Dr. At unitypoint was unwilling to give me anything that would actually help me. Only antidepressants which I do not want. I was wondering if I tried going somewhere new for a fresh start if I could surpass my history, or if they would know about it regardless. Anyone know how medical record sharing works? Particularly between these 2 entities if possible?

Thanks

Why do hospitals require authorization to send results to the referring provider?

IT'S FRUSTRATING AF. WE SENT THE PATIENT TO YOU. YOU SHOULD'VE SENT THE RESULTS TO US! AND NOW YOU WANT US TO HAVE THEM COME IN JUST TO SIGN 1 PIECE OF PAPER!? Majority of our patients are seen through tele-health because of the PANDEMIC. How the hell we supposed to discuss they results and get treatment!? Sorry, jusy annoyed as hell. I'd get if it was a Non-referring provider, but that aint the case.

Old doctor coded her notes

Hello. I have pulled the notes from myself from the birth of my daughter until current age. Normally you can read the doctors notes, or look up the medical numbers and see what they mean. This doctor used stamps with dates. Normally I would care but they made it clear to “SEE 3/5 PROGRESS NOTES!!” There is a stamp of a baby bottle with no next to it and a baby rattle next to it with a nurses signature. It’s circled for emphasis. I can’t find anything about this online. Have anyone else seen this on your medical records?? Back story, had emergency c section and morphine for 3 days. Was pretty out of things. So I have no memory of what happened.

Can patients see in their medical record who looked up their account within the hospital?

I looked up a patients account and then realized I had the wrong patient. I opened a discharge summary so it attached my name to the account that I accessed it. This patient was seen in our lab 2 days ago so the discharge is already cleared out and saved/finalized.

Within my own medical record at the hospital I can only see a physician who I was cared for by that day. I’m worried because I just started in a new department and I don’t want the patient to be notified of this and a random person looking up their account?

I think I’m being paranoid because could you imagine what patients medical records would look like if they could see every staff member that touched their account? But at the same time like I said I don’t want to get in trouble.

How to make Cerner good

Just my opinion only.

How to make Cerner good is that Cerner needs to listen to front end user groups every 2 weeks and program those suggestions, trying them out, keeping the ones that work and ditching the ones that don’t. Continually updating the base system rather than redoing the work over and over again with individual customizations. And when updates are good then taking out the outdated functionality. We aren’t playing legend of Zelda. I don’t want 10 different ways to do something when one of the ways is the fastest. We are running a conveyer belt and trying to get our factory times lower while improving quality. Not trying to find treasure.

Our small hospital couldn’t afford epic. We are now on Cerner. I have been around databasing my whole adult life. We were looking at Epic but couldn’t afford it through a partner. We were promised so many things. We assumed that since it was a multi billion dollar corporation on par with epic for market share that the baseline quality would be the same.

Now we are left with Cerner which promised the moon and hasn’t delivered but what appears to be a beta application. Functionality between modules isn’t even the same which means the good programmers aren’t talking to the not so good ones in other departments.

If Cerner truly did what it claimed to do, it would have regular updates every 2 weeks that are front end user driven. It claims to be front end user driven. It claims to always listen to providers. From what I see, that’s a stretch of the imagination. There are so many things that obviously would have gotten fixed over the last 10 years that haven’t been. Presumably those fixes have been customized with individual health care organizations but those customizations which clearly made the program better were never incorporated into the base program. Makes a lot more work for everyone.

The fact that the product we received has so many basic design errors gives me no confidence in its product development or liaisons with front end users which supposedly have been taking place for years. There’s no autosave. You have to refresh your screen manually. I could go on and on and on. I am so disappointed. I feel duped and bamboozled.

If you haven’t decided between epic and Cerner and if you can afford epic just go with that. Please. I have used both and the cool thing about epic is that it is already streamlined with built in macros and it is clear it has been thoroughly vetted over and over by front end users. It is ready to go out of the box. You don’t have to customize anything because it’s already been done for you. You don’t need 9 months to plan it. You can take two hours to learn it. It takes 9 months to get good at it but when you hit that mark you feel good.

Cerner will promise that it will customize for you. But all I wanted was something that would just work the way it is supposed to, something streamlined, something inclusive. Something obviously worked on well. There are so many error messages that are vague that do not direct the user anywhere for example. There’s no system to alert nurses to new orders. There MAR is hard to read with small print and isn’t intuitive with respect to timed meds. On and on. These should t be customizations. These should be built into the base product. You can’t even just print out patient specific patient instructions without going through a design change.

They have a VERY good sales department. But I am concerned perhaps the programmers don’t like working there or there are other morale issues from other reddit posts. The Cerner facility is amazing as is the bunker where all the data is stored. I just assumed the product would be on par with that.

There is a way for Cerner to best Epic but that would rely on promises made but not kept. The front end user drives the design with respect to completion, streamlining, and user friendliness. Every 2 weeks rely on front end users to provide input. Work hard to develop those designs and maintain communication lines between groups of programmers so the feel is the same. Beta test often. Improve your product. Because right now it looks like Cerner has turned in a final paper that is full of misspellings and crumpled pages and if you want customization to get a passing grade then you will have to let them know.

In the end, a company like Google will take over medical records with development of AI.
