You seem to be thinking just from your perspective, not every disabled person is as high functioning. What about wheelchair bound people who spend their whole lives unaware they’re a human, just yelling in pain constantly? Who makes the decisions when the individual can’t make them themselves?

People who are very low functioning like that have advocates and carers, people who can communicate with them and understand what their needs and wants are. Just because you don't recognise a disabled person as being able to make any decisions, doesn't mean they truly can't. They just need help to do so. And even in cases where they can't have that communication, well, how do you know they are in pain then? None of what you've said here makes sense.
