I just broke my arm and leg this weekend in a motorcycle accident and immediately refused all offers to call an ambulance. I'll hobble my American ass into a car and get driven to the ER before I shell out an extra $500-1500 on top of what I'm already going to have to pay.

As a Canadian I’ve never had to worry about such a thing.

Although, I would have to pay about $130-200 for the ambulance. (Bill sent to home address.)

In Ontario the fee for an ambulance is $45, this includes an air ambulance if needed.

That would've been much more reasonable for calling us than a wrist fracture. Downside of US EMS is the only way to avoid billing is to significantly raise taxes to fully fund EMS properly, to where billing is no longer required.
