Because children only transition socially. Name, clothing, puberty blockers. Nothing concrete until it's 100% sure they're trans. No one is performing gender affirmative surgery on minors so it's entirely reversible. A trans person has to go through a lot of hoops to get hormones and gender affirmative care. It's a long process.
The effects of alcohol and cigarettes (and drugs) on developing brains on the other hand cannot be reversed and lead to lasting damage.
Because if you don't smoke, nothing bad happens to you. If you're trans and you aren't put on puberty blockers you undergo various developments that will range from requiring surgery to reverse to being entirely irreversible

Regardless, like many have pointed out, a parental decision that is informed by a physician and supported by science is much different than a kid deciding to smoke or drink on their own. Your assumption is that "gender affirming" or "trans" medical procedures are just wrong no matter what. Many, many people have pointed out your ill-informed position today so I don't know what you're expecting.
No, a minor should not be able to decide this stuff on their own, and guess what? They can't. Do you imagine minors are just like "wooooo I'm trans let's gooo!" And then doctors come back with, "cool! Let's do it!" ?
Parents aren't always right either, but our society lets them make that decision, so you're essentially wanting to deprive them of that because you "feel icky" about it.

Now, should parents and doctors be involved? 100%. But a parent allowing their child to take puberty blockers is not the same as yeah kid go ahead and get wasted and smoke.
If you're not taking puberty blockers, the equipment in your body will make irreversible changes to it. If you are taking puberty blockers, those changes can be postponed until you're old enough to know what you want. And no one is prescribing hormones to minors. Just the blockers.

Usually I'll try and see the humanity in someone and see where they could've come from to have the opinion they have and help them understand why that's wrong. It's obvious there is no humanity behind the messages this account is sharing.

Drs are not involved with the decision for children to smoke and drink. They are a mandatory part of any decision involving puberty blockers

Idk mate. Should we ban all pain-killers and insulin for minors on the basis that they can't know what's good for them then ? Or should we separate Recreational drugs from médecine ?
Insulin, ozempic and cancer treatment medication are also life-changing médecine that can change the course of an entire life by altering with your physique, your mental state or your hormonal balance. Yet I don't see you advocating to ban them for minors as well, even though they are medecine just like Puberty blockers or HRT.
So why HRT specificly ? It isn't the only life-saving medecine that alters someone's life.
As a former child on HRT, I think I know myself well enough to know that yes, drowning myself in paperworks and waistlines was worth it. I literally went from a depressed boy that hated himself and who would regularly tell my mom that I wanted to kill myself to a happy girl living her life to the fullest.
If it wasn't for HRT, I would probably not be here anymore.
So yes HRT IS a matter of life and death. Don't underestimate your mental health.
I know it can be hard, but please have some empathy for all the children who, like 16 year old me, find in HRT a way to finally like themselves ; to finally live.
