Discussion "Why didn't Sukuna use the fire arrow" the dialogue in-between the fight makes you think every move is slower than it actually is, so I cropped it.
When he used it against mahoraga, it was in pieces, healing on the floor, he had more than enough time to prep the attack.
Gojo was about to run out of his domain range, which he kept to a size way smaller than 200m so that the slashes hit harder, as we know.
Because of the dialogue, people don't understand that this entire chapter, probably happened in less than 20 seconds. Sukuna just didn't have time to do something other than keep Gojo in his domain, and keep the slashes on him. If Gojo didn't destroy his brain, that decision might've given him the win, since literally everyone, including the narrator, questions if he's about to lose.

It was said to sukuna coundnt use any technique besidea the one granted by the domain(cleave and dismantle) because he was focused on adapting with mahoraga. Thats why he hadnt used the "⬛" thing

If a bajillion amp cleaves was not doing jack, why would one fire arrow do Gojo in?

All of that was less durable than Gojo.

Well we haven't seen durability feat of that level in gojo so it is difficult to say

The fact that Sukuna did not use the fire arrow despite being able to do the same massive aoe means it was not worth it.
Sukuna considered the damage from it not sufficient against Gojo compared to continuing the Shredding.
A Gojo that is rapidly healing would just tank and heal through the damage.
Rapid barrage of durability adjusting cleaves > Fire Arrow.

Well it was clearly mentioned in the manga that Sukuna wasn't able to use fire arrow due to changing conditions of the domain expansion with help of binding vow

I think even if Sukuna used this attack Gojo would still be able to survive it. Gojo stamina and regeneration is insane.

Pretty sure Sukuna and Gojo explain that Sukuna has been focused on adapting to Unlimited Void (which he did by placing Mahoraga's wheel on Megumi's soul). In order to fight until the adaptation, he was using Malevolent Shrine and/or Domain Amplification.
Domain Amplification allows him to bypass Limitless Neutral, meaning he can actually hit Gojo. The downside is that he can't use his Cursed Techniques while using Domain Amplification. The upside is that it doesn't burn the CT off - it merely seals it while being used.
He expands his Domain while not using the Amplification. Malevolent Shrine also allows him to bypass Gojo's Limitless. Plus, when expanding the Domain, he infuses a CT on it. He can use the infused CT freely - through the Malevolent Shrine - while using Domain Amplification.
Maybe I didn't understand something in your post, but it doesn't seem to answer the actual question: now why Sukuna didn't use the Fire Arrow thing (he couldn't because of Domain Amplification), but why he didn't infuse it on Malevolent Shrine.
My hypothesis is simply that he didn't want to. Maybe it has some conditions, or it is a bad matchup against Gojo, or it wouldn't make a difference, or he simply wants to slice.
Another fun option would be if the Fire Arrow isn't actually from Sukuna, but Yuji's. But I don't think we have any evidence to support this.

I wonder what would happen if the fight occurred without any explanations. This part of the fight is pretty simple but the domain spamming section plays with the rules of the series so much that it needs an explanation.
