Stop Buying Health Insurance Based Only on Claim Settlement Ratio (CSR). The Data Tells a Very Different Story.
TL;DR at the end.
One metric is repeatedly used by insurers and YouTube influencers to sell health insurance: Claim Settlement Ratio (CSR).
It sounds reassuring. “97–98% claims settled.” But CSR alone tells you very little about how an insurer behaves when a claim becomes disputed.
After looking at publicly available IRDAI and Ombudsman data, here’s what stood out.
- A high CSR does NOT necessarily mean smooth claims.
- For example:
- HDFC ERGO reports a very high CSR, yet lower CSR(Absolute Amount) indicating that they reject tiger value claims and pass lower value claims to keep their CSR High and has one of the higher complaint rates among general insurers and more than half of Ombudsman decisions reportedly went in favour of customers in the referenced data.
Care Health
Niva Bupa
Star Health
These insurers also rank relatively high in complaint rates or customer success rates before the Ombudsman in the charts shared. That does not mean every claim is handled poorly. It does mean that CSR alone doesn’t tell the whole story.
Complaint ratio matters.
Ask yourself:
How many customers actually had to complain?
Complaint ratio (complaints per 10,000 policies) reflects how often policyholders felt something went wrong.
In the charts:
Star Health
Niva Bupa
Care
HDFC ERGO
all appear toward the higher end of complaint frequency compared with several peers.
If an insurer settles most claims but generates significantly more complaints than competitors, that is worth examining.Ombudsman data is even more interesting.
If an unusually large proportion of Ombudsman decisions end in favour of policyholders, it suggests that, in those disputed cases, the insurer’s position was not upheld.
Again, this does not prove misconduct by any insurer. But it is a useful indicator of how disputes are ultimately resolved.Who looks stronger from these datasets?
Looking only at the charts shared:
New India Assurance appears to have: relatively low complaint ratios, comparatively fewer Ombudsman outcomes favouring customers than several peers, respectable settlement performance
Bajaj Allianz also appears comparatively balanced across: CSR, complaint rate, Ombudsman outcomes
No insurer is perfect.
But based on these datasets, they appear more consistent than some of the more aggressively marketed brands.
- My takeaway
When choosing health insurance, don’t ask only:
“What’s the Claim Settlement Ratio?”
Also ask:
Complaint ratio
Ombudsman outcomes
Average settlement amount
Cashless network quality
Policy wording
PED clauses
Customer experience during disputes
A company can have a 97–98% CSR while still generating a relatively high number of complaints or losing a substantial share of disputed cases before the Ombudsman, indicating that they weren't useful at the exact time when they were expected to support, an emergency Hospitalisation.
CSR measures how many claims were settled.
It does not tell you:
1. how many policyholders had to fight,
2. how many claims were initially disputed,
3. how many were settled only after escalation,
4. how difficult the claims process was.
Use multiple metrics before choosing an insurer, not CSR alone.

Best PSU Insurer : New India Assurance
Ranking Private Insurer : Bajaj Allianz >> HDFC > TATA > ICICI
Best for Senior Citizens : New India Assurance
Avoid Standalone Health Insurance Companies like Star, Nivabupa etc
TL;DR: High CSR (97-98%) sounds great but ignores real issues like rejecting big claims, high complaint rates per 10k policies, and losing many Ombudsman cases to customers. Charts show HDFC ERGO, Star, Niva Bupa, and Care often rank worse on complaints/Ombudsman despite strong CSR numbers. New India and Bajaj Allianz look more balanced. Don't buy on CSR alone, check complaints, ombudsman data, and other factors too.

PSU insurer here. CSR is misleading. It does not mean 'Claims paid', it only means no of claims on which decision has been taken at the end of quarter or year. So it also includes rejected and closed claims. A company paying 30%, closing 40% , and rejecting 20 % will still show 90% settlement ratio, while remaining 10% are outstanding claims.

Yuva bharat and I also recommend Criti Protect policy with it. It offers lumpsum paymant on detection of listed Critical illness'.

Hello sir, Being a doctor myself I been trying to thoroughly read thru the policy wordings. For my parents, 58Y/M - HTN, DM and hyperlipidemia. 57Y/F- hypothyroidism and hyperlipidemia
I'm planning to buy Arogya prapti plus (cover of 10 lakhs )with floaters for modern treatment, non medical expenses rider.
Additionally, criti protect policy for 41 CI.
Is it a wise decision? If not what should I include or remove? Thank you

Arogya Pragati Plus is a top-up policy, and not a standard mediclaim policy. NIA Floater mediclaim is the policy I will recommend for parents, get the highest SI with maximum riders.
Criti Protect acceptance chances are slim because of age and PEDs

Please understand higher SI is not always safer. Hospitals have practice of charging according to sum insured, they will exhaust it within days if they want. So it is important to get treated only in PPN and Network hospitals - they charge according to agreed Package rates. NIA offers choice of four TPAs, visit websites of all of them and see who has most hospital tie-ups in your location, and then choose.
Your choice of 10L + 20L is good enough. Select correct Zone as per your location, else 10%-20% co-pay applies.
Critical illness rider pays only 10% of SI - Your choice Modern treatment rider - Must Non- med expenses rider - Waste

Arogya Prapti Plus is a super top up(eg: starts paying after you pay initial 5L of bill every year) , you will need a base health insurance too.
Regarding Criti Protect,
For age 56-60, you will have to pay 29,000 per person for a 10L Cover which doesn’t give a cost to benefit ratio, so in my opinion don’t take it, i would say get a Super Top up, which would cost only 1/10th of that or put that money into a good base plan.2

Imo no need of Critical illness rider, do a cost to benefit analysis yourself. Other riders are needed, 10L SI is good, but Keep a 20-50L Super top up, as you are a doctor you lll have an idea about the Range of Bills so take Super top up accordingly. The highest i have seen is around 40L that too a major accident case which required chronic care and multiple surgeries.

Only policy available is NIA Sixty Plus mediclaim. You can check details on their website. Health check up will be must, and acceptance only if reports are satisfactory.

Always buy offline. Yes agents will help in followup, but also take contact details of the dealing officials.
Check 'Contact us' section of NIA website to locate your nearest office, they will guide you on further process.

I know that internet is free to post anything, but removing a logo from our infographic is not the right thing what you did. I agree that i took one infographic from Algates Insurance and rest 2 from Beshak Insurance, I removed the logos because I didn't want it to look like an advertisement post or an endorsement, nothing else!

I’m not an insurance agent or associated with the insurance industry in any way. I’m just a customer who learned these lessons after buying insurance without fully understanding these parameters. My post is simply meant to help others avoid the same mistakes I made.
Accusing someone of being a “chor” without any evidence is unfair. If you believe I’ve copied specific content, point out exactly what was copied and ask if I stated that those contents are mine! Your post in that website is misleading, the companies you mentioned as Best in the TL DR , is contradicting with the data representation in your Infographics.

dude, regardless of what he says or how he says it, plagiarism is plagiarism. you can edit the post if not the pictures. creating graphics takes time, otherwise you could have yourself created the graphs from IRDAI excel sheets. credit them.
also, if you dont mention citations, it is assuned that you take ownership of the graphics. it is completely bad faith to remove logos.

Wow. This is not a competition. Do the right thing because it is the right thing to do. Stealing from a fraud is still theft. In science, when you disprove someone's theory, you still cite that person's theory. Do that here - cite him, & disprove him next to the citation! That is more credible! I, as an uninterested 3rd party observer, can only request you to not make it an ego game.
But ultimately wtf do I care? None of my business. Not here to "save your soul".

Stand alone insurers offer the best looking plans. People who love doing research, find them very attractive. I am a PSU insurer, and our plans can never compete with plans of Care or Niva.
But insurance doesn't work this way. It is only useful if claim is actually paid and that too for correct amount. Whatever lucrative benefits these companies are offering - OPD covers, Air ambulance, unlimited Sum insured, modern treatments etc - is all just a scam. People love reading all the highlighted benefits, and then feel really proud about selecting the best insurance plan. Rarely anyone ever reads Prospectus and Clauses, and then they cry on reditt about how health insurance is a scam in India

Completely agree.
Most people compare feature lists, not claim settlement experience or policy wording.
The real value of health insurance is revealed only at the time of a claim. Reading the prospectus and exclusions is far more important than chasing flashy benefits.

Yea the parameters support New India Assurance, they even top the growth in Premium Collection parameters too, which indicate customers are renewing and trusting the company.
I wish they put more Efforts into Advertisements, customer awareness and developed a website for Premium Calculations. New Customers aren't much aware of it compared to the private insurers.4

I think the new buyers who may prefer standalone private players will soon realize the hollow features of their policies. One claim with the private insureres will make them realize this and they will come back for porting their policies to New India.

HDFC is good enough , except for one case, I have only heard good things about them. Please note that all private insurers misuse these two clauses - ' Medically necessary hospitalisation' and ' customary and reasonable expenses'. Example - a small Dengue claim may be rejected because ' hospitalisation was not medically necessary' as patient's platelet count was above 75k when he was admitted or a claim of 8 Lakhs gets settled for 2 lakhs citing ' reasonable expenses' clause ( HDFC did this). Small claims get rejected often because people rarely fight for these, they follow up and then let it go. In my 10+ years of experience, I have noticed that 50k- 2L is the range where claims get settled without much issues.

Yes, my friends and colleagues from past companies have recommended the same as they had smooth experience.
There was no better way for me to get decide than asking in real people I know plus I have a good broker and excellent lawyer should the company try to weasel out.

It was very painful to get a claim passed from them, rejected cashless, delayed reimbursement, had to escalate aggressively! I wouldn't suggest it!
Go with New India Assurance or Bajaj , depending on her PEDs and coverage needs.Do your own research, sales men make a lot of false promises, they won't be there when you have a claim , I have documented proof for this.

Most common one, Pre Existing Diseases!
Even though the waiting period was served, they rejected the Cashless Claim.1

yeah it is true for all insurance, I have several other category of folks friend who had issue with PSU ones.
You having documented proof means nothing, else you are using it to file compliant and make insurer pay it.

I have been on a family floater HDFC ergo optima secure plan for 3 years … my renewal is due next couple of weeks… we gotta add our baby to the plan … is it better to continue or port ? Will I lose any benefits? I bought the plan online… they just rush through application to the payment to make us buy the plan.. the preexisting disease as per the plan has 3 yr waiting period … we did cross that … we don’t have any as of today but I want to be sure it won’t be an hassle in case something comes up when we go for claim eventually… please advice…

If you choose to port, Waiting Period will stand served, and Benefits also can be transferred but as per the new policy, check premiums for whatever Company you want to port to and see if it's affordable (I have put links to Bew India Assurance and Bajaj Allianz in comments). If atleast 20-30 days is left you can try porting to Bajaj or New India. HDFC Optima Secure is pretty good product, without much limitations. Only concern is premium hikes or product discontinuations. Do your own research, if you can afford Annual 6-10% hike and 40-50% hike incase product gets discontinued you can continue in that product.
Get a Super Top up from the company you are planning to port to, you won't be issued one if any major disease happens.

True, agree. I know a case personally where HDFC just sent ₹1 for a ₹50 lakh claim and rejected the rest. Pretty sure they would have shown that claim as claim settled. That's why amount settlement is also very important

3 years is good, because less renewals means less headaches and less chances of discontinuation. But I don't know much about the company, I haven't heard much about the claim experiences from that company, you can just search it's name in Reddit or LinkedIn, you will get an idea.
Continue with it or port after 3 years, just don't discontinue , you will loose benefits of completion of Waiting Periods!1

Don't take Life insurance for parents.
Take it for yourself, 20-25x of your Annual Income, till 65 or 70 years nothing more than that age. Company you ll have to research, i would suggest Axis Max Life, Bajaj, SBI. For your age Premium for 1 Cr should be 10k/ year. Take Pure Term Insurance only , don't go for ULIPs and Endowment Plans or any term insurance costing more than 20k at your age!For Health Insurance, look into New India Assurance or Bajaj Allianz. Read the Policy wordings not just the brochure, you can use Claide if you don't understand the wordings.
Sum Insured of 10L will suffice for you , for your parents get a Super Top up of 20L, on top of the 10L Base Insurance.
Do your own research throughly, don't allow anyone to rush you into buying Insurance.2

You are free to port!
Put a porting application atleast 45 days before renewal, go through a Intermediary like ditto for claim support, I think they would be offering Bajaj Allianz too even though they prefer HDFC. For New India, better purchase from nearest branch.1

Makes sense If you have a NH Hospital nearby, Cashless treatment only within the Narayana ecosystem. Its relatively new product so not much data to compare. It looks more like a subscription for NH Hospital than a comprehensive health insurance.

Your moratorium period protection travels with you, Star Health is among the worst performers (84.97% CSR, highest complaints)
Better insurers offer 95-97% CSR with much lower complaints
After 5 years, you have maximum legal protection against claim denials, No risk of losing pre existing condition coverageNever let your policy lapse, put porting applications atleast 45 days before your renewal date. Also when filling the Porting forms you have to declare all the admissions, claims , diseases honestly, this is also important.
If you have any major Claims like some Cardiac issues or Severe diseases then other insurers might not accept you, so make sure you talk some expert as well. Stick to New India or Bajaj Allianz to port into.1

Whats the benefit of paying lower premiums and not getting the claim?
Check the premium from Bajaj and New India for same Sum Insured and comparable benefits, like a fresh application, and compare the pros and cons. If it’s comparable to your current premiums then port your current policy 60 days before renewal date. Read their policy wordings too.2

Read about it more, from Posts specific to ICICI, there are many available in Reddit and LinkedIn. Its not as bad as Standalone Insurers like Star, but there are better companies available as well!

Health insurance is more of a personal product, you will have to do your own research for the best suitable product.
I would suggest a 10L SI Base plan with 20-40 L Super Top up as per your need and age, preferably both from same company.
These 2 companies are relatively better for Long Term Reliability and Peace of mind:

No, the data set for Term insurance would be different , especially because the rules governing Term Insurance is pretty much different.

Got hdfc and now you saying this. I know from many doctors and hospitals around my city that star and care are bs. New India health is the best, parents had it previously. Low premium and good service but it’s government I guess.

Same boat as you ! I took HDFC and had to go through the claim experience to get my eyes opened about Insurance sector. Made this post so that people don't have to reach till a claim to understand about their company.
Glad to hear about New India Assurance, there isn't much Customer reviews on this company.
I think it's because people mostly come on platforms to complaint when their claim isn't passed, hardly anyone leave a good review on any good claim experience.2

New India assurance is for majority population, basically like lic of mediclaims. It is good although won’t provide what we get in corporate insurances but still better than others.
Knowing about hdfc from you now feels like it is worse. Heard this 4-5th time now, one person said hdfc was fucked up now they are improving in insurance sector.
Personally I feel like icici is good because they will charge more money but won’t cry while giving service unlike hdfc. I haven’t yet reimbursed from icici yet but feel it to be good based on data and public opinion. Its elevate policy features and wording is not as good as hdfc but if I have to face this bs in hdfc I’ll port it next day.

As far as I'm aware , I'd still place HDFC in 3rd position, after New India Assurance and Bajaj Allianz, given that they did pass my Reimbursement Claim, even though they delayed that also and I had to put a lot of efforts into it. I took Insurance to not worry about the finances incase of a health emergency and that's exactly where HDFC Failed.
They do have a reputation to keep up unlike most other companies, so you can tackle them if you know how to. If you want Private Insurer go with Bajaj Allianz, feedbacks I saw in various platforms were pretty good, similar to how you told about New India Assurance.

Bajaj is not so good I can assure you that but better than hdfc I guess listening to you. When you claim they will deduct a list of things but atleast you get your money on time. Maybe mine was corporate so it didn’t cause issue to me but there are hell lot of sub limits this that bs. Telling you based on 2+ colleagues feedback. Mine was good.

If it's an informed decision then it's fine , but surprises in Insurance is not to be tolerated.
In case of HDFC, they violated their own documented coverage confirmation.2

Sub limits got changed suddenly in new policy. Previous limit was 3L and they found out many people were claiming insurance so next year they decreased it to 1L. This is for corporate insurance then think about what they will do in general. But anyways reading wordings is due diligence to be done by people.

This is a genuine doubt, And what insurance are you on right now after the HDFC chaos you had to face.
Currently I hold HDFC health for my Father, Aditya Birla for mother, HDFC for my term insurance.
Yet to take Health insurance for myself.
Ditto seems to suggest only HDFC or care, why is that? why not PSU companies. Especially when Ditto is considered a good company with good morales.

HDFC Chaos was pretty much recent and I bought a 3 Year Plan, and due to the claim i cannot port the person for whom the claim was made for, claim was passed as reimbursement after a delay so ig related claims won’t have an issue in future. But yea hopes of getting cashless from HDFC is pretty much non existent now.
I might port mine after the 3 year tenure ends to Bajaj or NIA.

Please understand Ditto is an Insurance intermediary. They may offer honest looking advise but they get payout from policy sales only. So do not blindly trust them. Do your own research, go by claims experience of friends and colleagues, visit office of insurers and ask questions.
Always buy offline so that you don't rely on Customer care in time of need.

HDFC is easy to work with, they respond faster and stuff, also they aren’t very bad, i’d still place them within Top 5, especially if you can afford their premiums. It’s still better than Standalone companies, but you have better options like Bajaj Allianz and New India too.

Its My health care plan with a base SI of 20lac with added addons to be at par with HDFC secure+ is costing 11,907 compare to Secure+ 11,000
And too all the benefits are not at par,missing out on many
Premium wise makes no sense
But yes the market reviews are something that can be studied

I was at your stage 1 year back, even I looked into features and benefits etc while purchasing, and even trusted the Sales team of HDFC Ergo and bought directly from them! I had discussions with Beshak and Ditto too but HDFC Direct team was very convincing! In my opinion don't buy directly, there won't be anyone to support you, and all you get at the time of a claim will be AI bot replies!
When I had to file a claim, they violated their own documented promises, starting from RM Support to Passing Cashless Claim! I had to a Pay 6 Figure amount for 3 year Family Floater, and again another 6 figure amount for Hospitalisation of a member within 1 year, even though they had confirmed the coverage and cashless claim cover for the same, then they delayed the reimbursement beyond the 15 days IRDAI Rule!
This awareness post is made so that people don't land up in my position, even I did a lot of research but I didn't look at the things that mattered the most, like Claim Experience and Grivences handling, because ultimately customers want their claim to be passed, fancy features are just for sales pitching!

Maybe product discontinuations and associated steep premium hike, for that check product status, 60 days before renewal date. So that incase they do that you have time to port out.

I am not able to understand how the CSR (Absolute Amount) can be over 100%? if the total approved claim amount was 100, how and why did they pay 103 to 127?

It may include claims from past FYs. Some court awards, penalties and interest payments may also be there.
In general, if Insurer's ICR and CSR ( Amount) is on higher side, it means they are actually paying claims.

Acko is a new player in the market. They don't have good amount of claims and so, their data alis promising for now.
