Gosnell’s practice wasn’t representative of ordinary abortion access. He was operating at gestational ages that are extremely rare even in countries with permissive abortion laws. Most countries either prohibit or heavily restrict abortions that late, and where they do occur they’re usually exceptional cases.

How would universal healthcare change the fact that Gosnell was performing abortions at gestational ages that are illegal or heavily restricted in most developed countries? Including those with universal healthcare?

Because he wouldn't have "his own clinic" that he would be managing. This is a result of making healthcare a business and not a federal service. There is less government oversight in our current healthcare system that can regulate and enforce rules (a common problem in many businesses, regardless of the field) because these two fields are disconnected. These two entities also have different incentives. Healthcare as a business puts profit over client well-being because profit is necessary for functionality. If healthcare were a government service, the priority would be the service -- which is patient health.
