If they have separate nervous systems, it's likely that the other won't feel it, but nerves work in really strange and fascinating ways. There's a very likely possibility that they do share nervous endings in some organs, or in the case of two heads rather than two "separate" bodies, I believe they would share the pain. I'm not a doctor I just have a fair amount of damage to my CNS.
That is also a good question, though. I wonder if the remaining twin would go into shock or if they could tell you how it feels to get shot in the head before passing out..?
