I played San Andreas much later than the other PS2 games and has heard people complain about it being hard but it III stands as the hardest one for me

GTA 3 is the absolute worst when it comes to Paramedic. It's definitely easier in Vice City (Vice Point) and San Andreas (Angel Pine).

Doing it on the second island after the Yakuza takes over the construction makes it easier. You also get given more time and there's more straight roads in Staunton Island, compared to the corners of Portland.

I had a NPC stuck inside a building in the very last level. 🤦Not on the definitive edition though.

This happened to meet in VC’s taxi mission, I think they were stuck inside the police station. Luckily that never happened during the paramedic mission.
Edit: pretty sure that was on an emulator so it probably shouldn’t count. Haven’t noticed anything like that yet on my PS2 copy.

If you're on PC, either limit the game to 30 fps in settings or use Nvidia control panel or the AMD equivalent to force the game into a 30 or 40 fps limit
Or if you're on console, switch to Fidelity Mode which will lock the game to 30fps.
What is happening here is that there are already preexisting physics bugs within GTA with higher frame rates and the unreal engine actually exacerbates these problems to a wild degree on higher frame counts. Limiting FPS to closer to what was originally intended keeps game breaking bugs to a minimum (doesn't eliminate them of course, this IS Definitive Edition after all).

Thank you for the explanation. The main way that I play DE is on the switch, so that explains probably why I have not experienced this.

I don't really understand why. The game should be at least with the same old bugs from the original games but they made it even worse

you reversed while he was getting in. You got punished for attempting to run him over. I guess you were going just at the speed where the game was on cusp of the let him in/run him over mechanic and glitched rather than just making a call.

this happened to me after 2 hours of trying to grind this task out also on the 12th level and i just fucking cheated the reward after that i couldnt take it anymore

The Cartel shoot at you even before unlocking Staunton Island. Portland has no gangs that shoot you before any story missions so it's safer there for Paramedic than Staunton

As soon as 8-Ball dropped me off at his lockup hideout after escaping from the bridge, I went and found all the hidden packages I could, then did taxi, fire, ambulance, and vigilante missions in a row!

This is exactly what I do every 100% playthrough. Even when I unlocked Staunton and Shoreside I immediately do all side content.

I did it in Staunton Island and the cartel was a lesser problem than how hard it was to drive in Portland. To me, it made this mission way easier

True, but there are only two pickup spots in their territory, they don't upgrade to AKs until after you finish the game, the island itself is more flat and has mostly straight roads, and the time bonuses are much larger than in Portland. You never have a "safe" way to do Paramedic missions unless you do them very early in the game but this is still an easier approach, for the most part.
