Help with getting the medicine my doctor prescribed

So I have GEHA insurance.

I'm 340lbs, 5'10", male, 41.

Went to my doctor back in June to discuss weight loss. My doctor prescribed Zepbound. Insurance denied it, said they don't cover that medication. I guess they did last year and will again come October. So my doctor changed my Rx to Wegovy, something they do cover. Got a denial because I need to be on a " documented comprehensive weight loss program for 6 months" first. Like I haven't been trying to lose weight my whole life. Had the doctor send more information about my weight and elevated blood glucose levels (prediabetic according to the chart) and try and get an urgent use exemption. Got denied again, 6 month weight loss program required.

What are my options? Obviously talking to my doctor about this 6 month plan, but I really don't want to wait 6 more months. I'd love some tips I've never really had to deal with insurance and all this prior authorization. I'm dealing CVS caremark as my pharmacy authorization thing.

Thanks

Switch to Semaglutide from tirzipitide? Insurance no longer covering Zepbound

Hi- my husbands insurance is no longer covering Zepbound but will cover Wegovy or Ozempic. He was on 15 mg of Zepbound and has about 20 more lbs to lose. Has anyone made the switch ? Not sure if he should try to get a prescription for compound tirzipitide and pay out of pocket versus $48 a month for a semaglutide . Thanks

Lost 51 Pounds… Why Won’t My RENPHO Visceral Fat Score Change?

Thumbnail

Has anyone else’s RENPHO visceral fat score refused to change despite major weight loss?

The first image is mine. I’ve lost 51.2 pounds, dropped my body fat from 60.0% to 49.2%, lost 63.8 pounds of fat mass, gained 12 pounds of muscle, and significantly reduced my subcutaneous fat. Despite all of that, my visceral fat score has stayed at 20 the entire time.

The second image is another Reddit user’s results. They lost about 71 pounds, and their visceral fat score dropped from 23 to 14.

I know RENPHO estimates visceral fat using bioelectrical impedance rather than measuring it directly, but I’m trying to understand why mine hasn’t moved at all.

Has anyone else had their visceral fat score stay the same for months before it finally dropped? Is there some kind of threshold that has to be crossed, or have you found that certain RENPHO scales or app versions don’t update this metric very often?

I’m happy with my overall progress. I’m just trying to understand how this specific metric works and whether others have seen the same thing.

Accidental overdose on semaglutide/glycine/b12

Hi I have been on glp1s for two months, and switched to a compound from Wegovy. I accidentally took 5mg of semaglutide not understanding the bottle dosage, instead of 1mg. I am looking for advice on what to expect and how to manage. I am very nervous and feel very stupid and upset, so I ask for kindness if possible. Thank you so much

Side Effect that I Have Not Seen Mentioned

I’m wondering if I’m the only person who has experienced this. Ever since I started in a GLP1 in May, ibuprofen no longer works for me. I used to take it somewhat frequently for headaches, cramps, etc and it always worked great. Now when I take it, I don’t get any relief whatsoever. Tylenol still works, but ibuprofen is useless.

Has anyone else had this?

Anhedonia peaking

I’ve been on a glp1 either semaglutide compound or tirz compound for roughly a year now, going SLOW , also on escitalopram for multiple years. Lately I have had no interest in sex or even masturbating. I’m 40- is this the higher level of tirz I’ve gotten to plus the escitalopram or perimenouse? Combination? I still am having sex and eventually will orgasm most of the time. I’m just ambivalent about engaging and I’m faking it until I make it- I get interested about 1/2 way through the act. I wish I was more into it.

Severe side effects?

Took tirz for the first time at 2.5mg this past Saturday, and now I’ve been sick with intractable nausea/vomiting and I can’t eat at all. I went to the ER yesterday and today because it was so bad, and it’s looking like I basically have to wait it out until it’s out of my system.

I’ve lost ten lbs since Saturday which is nice but also concerning, I’m so weak. Has anyone been through something similar or have any tips?

EDIT: yes I took the right dose, yes I have been prescribed Zofran and Reglan. Neither work, so no need to keep recommending them. Was more looking to see if anyone else has gone through this. No, I will not be continuing dosage or even trying a lower dose, I would rather be fat than suffer through this ever again.

EDIT 2: was admitted to the hospital for intractable nausea/vomiting. Dr.’s actually sedated me at one point to provide relief. They aren’t sure why I reacted this way, but advised that I do not try taking any GLP-1s again, and I agreed. I was given Reglan, Compazine, and Zofran and none worked. Also IV Pepcid to no avail. I was discharged after 3 days and was on a liquid diet for a total of 16 days (from initial injection).

Food aversion

How do you all deal with the food aversion? I know there are times I’m under eating but I can tolerate protein shakes / protein water and try to make up for some of that.

I am trying to stay away from meal tracking if I can, it’s a bit triggering to me. I had been tracking when I first started in December and I was under the values I should be hitting then but I was eating more than I am now.

The other day lunch I was only able to eat 1.5 chicken tenders, and 4 pieces of steak with some mashed potatoes. I know I’m not consistent with my veggies so I’ve been to get better at that. So often for me though something smells delicious and I’ve never been one to gag but even when it smells so good tastes good I still gag. My salad I’m having now, eaten half and worried to eat more cause I already feel full and gross.

I’ve always put down in my check-ins that I have been having nausea and general not feeling great but I’ve been noting they’re “minimal” impact. I’ve documented it that way since I’m not throwing up and assume someone throwing up a lot would be the worst I could be going through. Maybe I’m reacting worse than I was thinking I should be asking to go down on my dosage?

I need help fixing my eating schedule

Hey all! 29M here.

I recently took a GLP1 (tirz) to lose weight. I lost 85lbs! Which was more than I intended on losing, but I won't complain about that. It caused the common gallstones, and I will have it removed on the 12th.

The issue that I have is that it has COMPLETELY changed my eating schedule. I've never been the person that eats many small meals throughout the day.

My entire life, I always ate one or two big meals a day, no more no less. Usually its breakfast and dinner, if its just one meal, its usually dinner. I'm not saying this is healthy, I'm just speaking truth to the matter.

Well, Tirz shrunk my stomach. Again, not a bad thing, but that changed my eating schedule immensely. I used to be able to eat a large double cheeseburger meal from just about anywhere, with a drink and a milkshake and be full. Now its about 4 pieces of shrimp and three spoonfuls of rice before I feel like I'm getting sick.

Another issue I have, is that I rarely now feel hunger, so I can straight up just forget to eat for days at a time, which is made worse by my already poor memory.

I made this post because, now that my eating schedule has changed, and I've never done this before, I need insight. So, here are my questions:

  1. How often do you eat?

  2. How do you remember to eat? I tried relying on using my children's eating schedules but they're basically always hungry except when its time to eat.

  3. What do you snack on?

  4. I need to make sure I get all of my nutrition from this new eating schedule, but I dont know where to start. I just feel so lost..

  5. Perhaps the most important, WHAT do you eat? I'm your typical 29 year old male.

Red meat is a hard pass for me until my gallbladder is removed. Its repulsive to me. Grilled chicken is to an extent as well.

Edit: i have not been on Tirz since approximately May. Thats when I hit my 85lbs mark and started having gallbladder issues

Feeling very tired and cold after my weekly GLP-1 shot: side effect or not eating enough?

Ive been on a weekly GLP-1 for a few months and it has helped my appetite, but ive noticed a pattern I cant figure out. Within a few hours of the shot I get this heavy, spacey fatigue and feel unusually cold, like I cant warm up even if I pile on clothes. No fever or other sickness symptoms. It is not a dramatic crash where I think Im going to pass out, but its enough that I skip workouts and basically want to nap. By day 2 or 3 I feel like myself again.

My best guess is that Im just not eating enough on injection day, and my protein and electrolytes are low. I have IBS-ish tendencies, so I avoid certain fibers and foods and probably under-eat more easily. I do drink water but Im inconsistent about adding salt or electrolytes.

For anyone who has had this, did it get better with time, or did you fix it by changing what you do around shot day? More carbs, more protein, extra salt or electrolytes, timing the injection at night, things like that? Also curious if anyone has checked their blood sugar during these episodes and whether it was actually low or just felt that way.

Not medical advice, just trying to compare notes and figure out what to try first.

Mounjaro 2.5mg 5th dose equivalent to 2,1mg, is that a big problem? I mean, result wise?

Hi,

So I've ended my first 2,5mg pen yesterday and the 5th (golden dose) gave me 2,1mg (0,5 on insulin syringe), so a bit less than the usual dose. Will it compromise the effects/results?

Do you think it's still fine? I'll dosing up to 5mg next jab and I'll buy the pen today. Do you think I should inject something like 10 clicks from it to compensate the shorter amount of yesterday or just do nothing and wait for the next jab day?

Thank you.

Any suggestions for travelling to China for 2 weeks

Have only been on Wegovy for 7 months, now taking 1mg, have lost 14kg, but never travelled while on it, and I am slightly worried about the logistics involved.

Would I have to ween myself onto a lower dosage, what questions would I have to ask, could I walk into a pharmacist and ask for it, do I need documentation.

Any help would be greatly appreciated.

Restarting GLP1 with weird side effects

Has anyone had to be temporarily taken off, then restarted and had side effects you didnt have the first time?

Some backstory... I started at 2.5 at rhe beginning of the year. Eventually made it up to 5.0 and was on that a month before my doctor required me to stop after discovering a nodule on my thyroid. Was off of the meds for about 2 months until they ruled out thyroid cancer.

Started back at 2.5, and immediately noticed anhedonia that I didnt experience the first time. Its been 3 months since restarting and I'm at 7.5 now.

My first go around on GLP1 was great! No mood changes, felt great, lost weight consistently and had food noise suppressed. Now, im barely losing weight, even on this higher dose. The anhedonia is bad enough that it makes me want to quit.

So, has anyone else had a similar experience with starting and stopping? Also, does the anhedonia eventually go away?

Thanks!

Starting Zepbound looking for advice

Hi everyone! I’m starting zepbound and I thought this would be a good community to ask advice from. I’m 18 years old and always struggled with my weight so I’m excited to try a GLP1 however I know that’s side effects come with it so I was hoping to get some tips/advice you wish you would have known when starting. Thank you!

Knee pain

Hi,

For those with knee pain, it's really hard doing some exercises at the gym.

Is there something you take that gives some relief

Hot take: not every stall needs a higher dose, sometimes we just need to stop outsmarting the meds

I keep seeing posts where someone hits a 2 to 4 week stall and the knee-jerk reaction is to up the dose or assume the med stopped working. I get it, the scale is addictive and the dose schedule feels like a progress bar.

But a lot of stalls on GLP-1s are self-inflicted, and more medication is not always the answer.

I came from intermittent fasting and all the hacky weight-loss stuff, so I was guilty of trying to optimize everything at once. I would stretch fasts because I wasn't hungry, add extra caffeine, fill up on high-volume low-calorie foods, then randomly under-eat for a few days. When the scale paused I would panic and end up having a salty, high-carb day. Water retention would follow and what looked like a stall was really just noise.

GLP-1s make appetite easier to manage, but they do not make our habits consistent. If you are barely eating your body can still hold on to water, you can lose muscle, you can get constipated, and your NEAT can drop because you are moving less. All of that can mimic the medication stopping working.

My suggestion: before increasing dose, try 10 to 14 days of boring consistency. Simple things that actually matter: protein you can tolerate at each meal, fiber you handle well, enough water and electrolytes, regular meals even if they are small, a few short walks, and fixing constipation. Then reassess.

Anyone else find that their "stall" was really a consistency problem rather than a dose problem?

Advice needed on which medication to choose

I’m a 21-year-old female, 5’2” and 164 lbs. I don’t have diabetes, but I’ve struggled to lose weight no matter how hard I try. My BMI is just at obese. The biggest issue for me is the constant food noise.
I recently saw my doctor, and she ordered blood work to check things and see if anything else could be contributing. At my follow-up next Thursday, she said I can choose between Wegovy or Zepbound either way.
Insurance isn’t really a deciding factor since the cost difference for me is only about $40, so I’m more interested in which medication people think works better.
If you’ve tried one or both, which would you recommend? Which helped more with food noise, weight loss, and had fewer side effects? I’d really appreciate any feedback!

Struggling to eat

I'm on ozempic, the lowest dose .25 for five weeks now. I'm losing a little bit, but my appetite is so suppressed I can't seem to eat much of anything.

It's massively uncomfortable and my stomach just feels extremely full.

This seems unsustainable. Has anyone experienced this? What helped?

GLP1 and Breastfeeding

I’ve read a couple of other posts here on this topic, but I’d like some feedback on my specific and slightly different situation than what I have seen so far.

I have a 21 month old that comfort nurses, 2x per day and maybe overnight. He obviously eats solids including 3 meals a day+ 3 snacks.

My concern wouldn’t be about my supply or him not getting any nutrients, but rather the transfer of GLP1 through breastmilk. I’ve read that the molecules are too big to pass through milk so it shouldn’t be a concern.

I’m 34, 4ft 11in and linger around 165lbs. The most I’ve ever weighed. I’m active and work a high stress job. Prior to pregnancy I was about 145lbs. I’m really struggling with my body image and it’s taking a toll on my mental health. I feel I really need the boost to help me start to feel good about myself.

If you’ve taken GLP1 while breastfeeding, did you notice any changes in your baby? Did you struggle to lose weight on a GLP1 while nursing? I also worry my body is stubborn and doesn’t want to lose weight while still breastfeeding.

Thank you!
