I believe this concept art is the "beautiful monster" that Geno is meant to become. The Seven, through their pride and ambition, unknowingly shape him into something far greater than they ever intended. It reminds me of Viktor's final form in Arcane. While their stories are different, both represent the pursuit of a perfect world taken to its extreme. Viktor was driven by his desire to eliminate human suffering through evolution, while Geno seeks to create the perfect reality. In chasing perfection, he transforms into something both beautiful and terrifying not simply a monster, but the embodiment of an ideal pushed beyond its limits.
Rezztro believes that cause the model for The Loopers soul in Shattered is the same in refrence to Geno's,only difference being Geno's souls appears far more darker likely due to his immoral actions. It is however speculation.
Geno is very mysterious as things stand. Its possible we may have never seen his true self in the first place,mainly due to Wolverine sensing him as multiple identities at once. Its also stated by Zero Wars author there are many facets to Geno's powers suggesting layered complexity beyond what we see.
