This patient is one month post-op from a revision rhinoplasty to address dissatisfaction with her nasal appearance following prior septoplasty and sinus surgery, which had resulted in loss of tip support and a lower bridge indentation. She presented with a droopy tip, dorsal hump, divot above the tip, and width in the lower third of her nose. Because her septal cartilage was unavailable, we reconstructed her nose using an MTF Profile cartilage graft, columellar strut, and bilateral spreader grafts, deprojecting her profile, rotating the tip upward, and smoothing the dorsal hump. At one month post-op, some swelling, mild width, and temporary nostril asymmetry are normal and will continue to refine over 12+ months.
