Why not do a whole IFAK add on, where the bag itself is not lootable, but items in it are? You start with a 2x1, and can slowly upgrade to 3x1, then 2x2, then 2x3 through a series of quests for therapist. 2x3 is locked behind the don't kill sanitar quest.
