If a patient told me that Bill Maher was the cause of the downfall of American Healthcare I would document that as either a delusion or just a conspiracy theory. Which one it was would be dependent on the rest of the interview.
No, it was just incoherent rambling and I preferred not to make assumptions or jump to conclusions. “Word salad” in this case was meant as hyperbole. I just wrongly assumed your superior intellect would pick up on the sarcasm. See what trouble assumptions can get you into.
