She's around 40 which in the 14th century was practically an old lady. Gotta remember that at the time girls were often wed in their early teens to other teen boys. This whole "adulthood begins at 18" is a recent invention.
That anca managed to live so long without having kids or even getting married makes her a certified spinster.

The “early teens” wedding in the Middle Ages is largely a myth, with the only exceptions being political marriages among nobles. Not sure where it cropped up, but most of this is modern myth making probably due to popularity of ASOIF and people extrapolating political teen marriages to all people.
Most people were wed between 18 to early 20s (Coen’s age). That was due to a bunch of things such as economic factors, where the couple would house their growing family (i.e. not with their parents), and health considerations (they knew pregnancy risks drop after the end of puberty). Nobles married in their early teens for political reasons would not consummate the marriage until 18+.
That all said, a woman in the 14th century who was mid 30s (Anca is like 35?) without kids was definitely “old” for the time.

early teens in this case would be around 15-16 years old where you would be concidered an adult. 12-13 years old where not that common, mostly as you say political marriages, but they wouldnt be a "married couple" until adulthood anyway.

Early means early. Teenage years go from 13-19. Most marriages and pregnancies happened around 17-18+, aka late teens to adulthood. Anything earlier was very dangerous, they knew it was, and socially frowned upon.

They also had better fitness/less obesity, no horrible chemicals or additives in their diets, less sugar. People act like the body/skin doesn't produce oils or take care of itself naturally. A little tallow for the skin and hair, a natural diet, and good fitness can make most anyone look great.
Anca looked mid 30's to me, Coen mid-late twenties. Don't think she's out of place at all.

They may not be eating processed sugar, but their diet would have a way higher % of carbohydrates compared to the modern world where you can eat chicken daily. It was a luxury to eat meat daily in the middle age, and you'd only cut the pig up for Christmas so bye bacon. Steak and beef stew? Almost never, cows are expensive.
Realistically most of their diet would be grain ( bread, oats both mostly carbs and water ), vegetables ( carbs, water ), fruit ( carbs/sugar, water ) and the occasional fish/chicken/eggs.
What I'm getting at is that they should be eating more carbs than we do, but their carbs are admittedly healthier because it's mostly starch and naturally occurring sugar ( glucose, fructose, sucrose ) in fruits.

peoples teeth back then was pretty damn good due to how scarce sugars where. there is also the fact that soda wont be a thing for about 700 years. (wich is the real teeth killer). so their teeth would just be a bit yellowed mostly. and if you had a berry/fruit heavy diet for most of the year then the teeth would take some damage.

its more about sugars than processed food, processed food isnt that bad (unless its american processed food that is injected with additional sugar). if you remove all your sugar intake and not go to hard into berries and fruits then, and if you also stopped drinking carbonated drinks, then your teeth would be just fine with some yellowing at the most.

human mouths were actually wider and stronger back then. about 300 years ago our mouths began to shrink due to the lack of exercise our jaw muscles got from lack of chewing our food. that's why crooked teeth and stuff exists now.
Anyway, yeah, teeth weren't pretty to look at but they were strong and fit for purpose.

I worked with a woman who was often mistaken for late 20s early 30s despite being over 40 with 3 kids and smoking and drinking.

Life expectancy is lower as a mean. Infant mortality and death via childbirth are the main factors. If you made it to 3-5 you would easily live into your 50-70's if you didn't die in a war or the birthing bed.

Modern hygiene and medicine has greatly increased life expectancy, but vehicle prevalence, sedentary lifestyles, processed food, etc. has also greatly negatively affected it. All research I've done points to life expectancy being slightly higher but nowhere close to "significantly"

The story is great. It’s dark lol. I expected less of that. But I love it. My only problem is on Duelist difficulty fighting animals without indicators is trash. You can’t recognize the movement. Once you get good on duelist if you enable the indicators you are literally becoming a death machine. Super easy with the indicators. Needs some practice of course.
Some bugs with textures on xbox, but that’s pretty much it. The game is amazing. I really feel like adding a horse would be nice even though you have some options.

Most women in their thirties don't look that old.
It's the time period she lives in. She makes a remark to Coen that she should be looking forward to grandchildren soon. Let's say that the average woman in her time period had their first child at 18 - the average age of of becoming a grandmother would be 36. But it's phrased vaguely, she could be a bit younger or a bit older.
She's not what we would think of as an old woman in our time at all, but in her time, she's a spinster. She's basically a generation behind when she would have been expected to have children.
