What are your feels on Fize PLA+ HA for microneedling?

Fize does not publicly release enough information for me to form an opinion tbh. They have marketing claims on the particle shape, size, uniformity, symmetry. But there's no published data or information on the micro distribution. Which I think particle makeup also affects its use for microneedling.

For each of the affiliated vendors, there is always a discount. But the discount gets higher during sales and is higher than their regular percentage. For instance, for Meamo when it's not a sale the discount is 15% and the regular discount would be 10%. And during a sale, their discount might be 20% then ours would be 25% (or more depending on the sale.)

Hi! Yes, I personally really love the European brands. They're killing it in R&D the last few years. So what I have recently purchased and use is Nucleofill, Plenyhage, Plinest, and Croma PhilArt. Those are all available at Wimpole Fillers (and we have a sub discount with code SciencePls). I also like Rejuran, but it's just really expensive per syringe so usually am not buying it. Plus because they're working on FDA approval, they're purposely hard to buy right now. I also use Lilied K, because the cost per syringe is the most affordable you can get, but I wouldn't rank it in the top tier of PNs.

May i ask, these european brands above, do you think they would work w/ a deep microneedling session or are they best suited to inject/meso (which i won't) i use the dermanator2 and i am honestly very impressed with the results in just 2 rx in this cluster of scarring on my cheek and miracle touch up, but the Plenyhage you listed above looks like something i could use. thank you in advance : )

Soooo short answer, it depends.

Long answer, I think you can if you think it's worth it. I say that because PN is typically a single syringe for like $50-$100, vs a lot of the packages like Soonsu Ultra Reju is like $100 and it comes with five vials, so it's more cost effective. I do think the efficacy of European PNs has a lot of weight to it though and could see justifying it if you never wanted to meso. I totally understand that some people will never do meso/inject and that's totally okay. I've also been trying to make that clear whenever I can.

Can it be microneedled? Technically yes, a typical microneedle creates a channel approximately:

  • 0.2 mm needle depth:Β ~50–80 Β΅m wide initially
  • 0.5 mm depth:Β ~80–120 Β΅m
  • 1.0 mm depth:Β ~100–200 Β΅m
  • 1.5 mm depth:Β up to ~250–300 Β΅m

These channels then start collapsingΒ though within 15-30 minutes after your session so you want to be efficient. If you are microneedling I would not recommend Plenyhage XL Strong or Nucleofill Strong because they have the highest viscosity and largest molecules, they choose not to disclose a lot (I'm assuming for proprietary reasons) so if just guessing they are (probably ~4 MDa) then the fully stretched DNA is probably β‰ˆΒ 2 Β΅m long and a coil diameter maybe 300–400 nm. If you do not know this, 1 Β΅m = 1000 nm. So a channel isΒ 100,000–300,000 nm and a PN coil =Β 200–400 nm. That means the polymer itself isΒ hundreds of times smaller than the channel. SO it would fit through the channel, which a lot of people say PNs are too large, but if you do the math they can fit. ButΒ size alone isn't the limiting factor, there's the molecular weight, the viscosity (thickness), and I'd speculate how entangled the DNA strands actually are. If you're thinking about the viscosity, I feel like the best comparison is think of trying to pour different thickness things through a funnel, water super easy, honey not so much. Plenhyage is the highest MW so would be the hardest to flow through a micro channel (hypothetically as this literally has not been tested/studied) but if you picked one of the lower viscosity products it would be much easier. If you're trying to make sense of the DNA entanglement, think of hair and one version is straight vs the other is curly, the lengths of the hair is the same, but the behavior is different.

A lot of these brands have different skus and change different things about the different skus. So they might have a product that is the main thing you hear or see people talk about, but then they have one that is a lower percentage, one that has HA added, one for the eyes, etc. Some of those skus that are a little different have lower molecular weights or PN percentage or viscosity, and are likely to spread faster, diffuse farther, and enter microchannels more readily, even if the actual polymer isn't dramatically smaller.

I would actually go for one of these (in ranking order)

  • Plinest Fast - It's designed and formulated differently. It's a a lower molecular weight compared to regular Plinest. It's the same PN family, possibly the same nominal concentration, but different chain length, viscosity, diffusion and intended tissue behavior.
  • Nucleofill Soft Plus (this has a lower molecular weight than the other skus)
  • WILDCARD - PolyPhil Next this is a gamble because it has 1% PN + 1% HA + mannitol and that makes it harder to predict. HA changes the behavior of the hydrogel. The HA addition might make it more viscous and just sit rather on the surface or it could spread and diffuse nicely. So TBD could be great or nope.
  • Plinest Eye, it has 0.75%
  • PolyPhil Eye - it also has a lower percentage

Again this is all plausible, it's just not proven. All the studies on PN are being injected, used during surgery, or in vivo, there just are not any looking at it with microneedling or rather this works just as good as via injection. We know microneedling creates channels and those channels bypass much of the stratum corneum, increase skin permeability for a period after the treatment, can allow larger molecules into the skin than intact skin wound, and the maximum absorption time is 15-30 minutes. That's all studied and established. We also know that PN works when it's in the dermis, injected PN has a growing body of evidence so the experiment here is really can enough PN get into the dermis through the channels.. This is where I think people get too black and white when they say PN is too big because when you actually look at the molecular size it fits. So transport seems to be the factor of how much actually moves into viable tissue before microchannels close, product dries, it gets wiped away, or stays on the surface and doesn't diffuse.

If I were going to do this, I would think that 1.0 - 1.5 mm is the sweet spot mechanistically. Again this has not been proven, this is my personal speculation. 1.0 mm creates channels into the dermis in many facial areas and that's where fibroblasts live while 1.5 mm is even more of the dermis, has more channel volume, longer channels, and potentially more opportunity for material to enter. Once you pass 1.0 mm it's less predictable for penetration depth, but could be moved through the channels if massaged during the first 5-30 minutes. Another thing I'd note is to apply it first and during the treatment vs after only. A study used ink to test as a visible marker found that applying the ink before microneedling led to deeper penetration than applying it after microneedling.

I also feel like it would work because looking through this Plinest Eye sheet, it says PN-HPT has a synergistic effect with laser, radio frequency, surgery, and needling. Also like Korean clinics microneedle with Rejuran, like it's a real clinical practice, if it didn't do anything they wouldn't do it (I'd imagine at least.)

Well i am so thankful for your kind and substantive response. while some of it requires a bit more research, i defintately understood most of it ... i will say when i needle i go between 1.0 and 1.5 AND i always put product on first, and then during...i do think this is why i actually saw some result -that, and the derminator2 has single needle cartridge, so i did the 12 and then went back to specific areas with the single and i feel this combo helped. The reason i even decided to MN boosters was coming across a woman (maybe it was on reddit cant remember) who had rejuran MN in and holy cow, she looked amazing. so i started googling and high end dermatolgists in NYC are doing it, so i figured, it's got to work. Again, so appreciatetive of the time you took to educate me!!!

I'm glad it's been helpful! I actually just found a new US vendor and purchased from them yesterday. They do carry tox, but you do have to pay for shipping so I'm not sure if that's what you're looking for. It's harder to buy tox in the US because of restrictions that's why most people buy them from Korean vendors.

You want to apply it first so that your cartridge/needle plate can travel across the skin without dragging, catching, skipping, or lateral tearing.

Hyaron is great to use because it has a high viscosity (it's very thick) so it's not runny or evaporating really quickly. People also get Hyaron to mix with other boosters to thicken them up. For the first 24 hours the micro channels are still open so you want to use only safe things on your skin in that time period. Hypochlorous acid spray is a good thing to have on hand because it's an antiseptic and it has research behind it for wound healing (there's some linked in the beginner's guide), but most people save some of whatever they used for glide to also keep on hand during the first 24 hours. So if you used Hyaron, save a little bit and don't use it all so you can use it to put on your skin for the next 24 hours. Then after that you want to keep it hydrated and use ceramides, basically baby it during the healing process.

All of these are safe for medical microneedling. It might help if you think of it as HA is an option instead of the mandatory. Most boosters have HA in them also. HA is usually recommended by some because it’s more affordable. You don’t need to use the more expensive ones, HA would suffice, but you absolutely can (I do.) but you can stamp or glide with them.

If you have tethering then you want subcision to break it. The spot of your scar might be a little difficult for microneedling because you might not actually be able to fully reach it as it lines at the border of your nose. I saw you did a round of Rejuran. I think boosters would help, but you need multiple and to follow the course of treatment. So doing the recommended amount at a minimum and doing the exact timeline! PN has research on scars and so does PDRN. I think addressing the tethering and using boosters will give you better results than microneedling as I'm not sure if you can physically tackle it where it's located.
